Job Summary:
The Multiple Complex Needs (MCN) Support Worker assists individuals who face unique and intersecting challenges in their daily lives, including mental health concerns, past trauma, substance use, limited natural supports, poverty, homelessness, or involvement with the justice system.
As a member of our MCN Team, you will play a key role in helping individuals achieve their educational, social, recreational, and emotional goals within their community. MCN supports focus on building connections, meeting individuals’ physical and behavioral support needs, and liaising with the Program Facilitator, Program Coordinator, and/or Program Manager.
The MCN Support Worker plays a crucial role in fostering community integration.
Hours and Compensation:
- Must be available on weekends
- Weekday hours may also be available (schedule based on individual needs)
Responsibilities Include:
- Provide services for people facing multiple complex needs.
- Plan, organize, implement, and evaluate community-based recreational, social, and educational activities.
- Encourage and facilitate participation in activities; provide guidance and instruction.
- Provide skill-building support in areas such as life skills, social skills, and employment-related skills; model appropriate behavior.
- Use positive behavioral supports at all times to ensure the safety of individuals served, community members, yourself, and other team members.
- Respond to crises or emergencies in accordance with established policies, behavior support plans, and safety plans, using a trauma-informed approach.
- Communicate progress, changes, or concerns regularly to the Program Coordinator and/or Program Director.
- Communicate with families or caregivers of individuals served.
- Complete shift reporting and documentation of supports provided and individuals’ progress.
- Maintain related records and produce reports as required.
- Participate in the creation and implementation of Person-Centered Plans.
- Follow health and safety standards.
- Administer medication as required, in accordance with established guidelines, procedures, and instructions.
- Record hours and shifts worked in the virtual payroll system.
- Participate in staff meetings.
- Attend professional development opportunities as assigned.
- Perform other tasks as assigned by the Coordinator and/or Program Director.
Qualifications:
- Post-secondary certification related to supporting people with developmental disabilities, or equivalent experience.
- Experience supporting individuals with developmental disabilities, the unhoused population, and people who use substances.
- Experience applying a trauma-informed, harm reduction approach.
- Comfortable providing personal care.
- Experience supporting individuals with challenging behaviors is an asset.
- Class 5 driver’s license, clean driver’s abstract, and access to a personal vehicle are key assets.
- Training in Naloxone administration.
- Flexible availability.
- First Aid Certification required upon hire.
- Clear Criminal Record Check.
