Provincial Homecare is seeking an Onboarding and Engagement Coordinator to lead the intake and onboarding experience for both new clients and new employees. This role is central to ensuring a smooth, welcoming, and well organized start for everyone joining PHC, whether they are a family bringing on home care services or a new team member joining our staff. The coordinator will also conduct home assessments to ensure client environments are safe and appropriate for the delivery of care.
Key Duties and Responsibilities
- Serve as the primary point of contact for new clients and families from initial intake through to service start
- Guide families through required documentation, consent forms, and service agreements
- Lead new hire onboarding
and completion of employment documentation
- Track onboarding milestones and flag any gaps to management
- Conduct in home assessments for new and existing clients to evaluate safety, accessibility, and suitability for care delivery
- Document findings and communicate any concerns or required modifications to the appropriate team
- Check in with clients and new employees during the early stages of their relationship with PHC to gather feedback
- Maintain accurate records in AlayaCare and other systems related to onboarding activity
Qualifications
- Experience in home care, health care coordination, HR onboarding, or a related field
- Strong organizational skills with the ability to manage multiple onboarding processes at once
- Excellent interpersonal and communication skills
- Comfortable conducting in home visits in the service area
- Familiarity with AlayaCare or similar scheduling and care management platforms is an asset
- Valid driver’s license and access to a vehicle
