Wendat Community Programs requires a full-time permanent Transition Nurse to work in our Psychogeriatric Program, facilitating transitions to living situations for seniors presenting with behavioural challenges. RPN with experience in Long Term Care and hospital settings preferred. Experience working with seniors with a serious mental illness which includes dementia and /or other complex medical and neurological disease processes. Good knowledge of behaviour management strategies and strength-based care planning. Must be able to travel across Simcoe-Muskoka. Reliable car and clean driving record required. Must be fully vaccinated for COVID-19. Proficiency in both French and English an asset. Monday to Friday 9 - 5.
Must be registered with the Ontario College of Nurses.
Wage band is $54,800 - $66,400, starting and full salary will be set based on experience.

Position Title: Transition Nurse, Psychogeriatric Program
Reports to: Director, Seniors Services
Position Summary:
The Transition Nurse facilitates successful transition from a person’s present living environment (i.e. Hospital in-patient, Retirement Home, private residence, etc.) to living in a Long-Term Care facility or other environment of choice, for seniors with behaviour problems.
Qualifications:
- B.Sc. (Nursing), R.N, or R.P.N. and must be a member of the Ontario College of Nurses
- 5 or more years of experience working with clients with various types of dementia, or other neurological conditions who are experiencing a responsive behaviour and/or family members/caregivers of these seniors
- Eligible to work in Canada
- Experience with hands-on care of the geriatric population in various care settings (community hospital/LTC/retirement home/etc.)
- Understanding of and ability to use concepts, principle and specific techniques of behaviour management with seniors
- Knowledge of the system of services for the senior population, in particular Long-Term Care facilities and Behavioural Support Services.
- Able to exercise good judgment where procedures and protocol have not been set and where decisions could have a serious impact on the life of another.
- Superior communication skills characterized by diplomacy, tact and negotiation skills to effect cooperation from different service sectors and influence opinion at an aggregate level.
- Valid drivers license, reliable car and ability to travel long distances across Simcoe-Muskoka region.
- Proficiency in both official languages an asset
Hours of Work:
The work week is 40 hours and will usually be worked Monday to Friday from 9 am to 5 pm. Occasionally, based on client need, it may be necessary to flex these hours. Additional flexibility may be required to allow for staff training, client/agency emergencies, special agency functions and other responsibilities.
Agency Responsibilities:
- Ensures at all times the confidentiality of client and agency information
- Conducts all matters in a professional, diplomatic manner
- Provides client centred care with respect to individual dignity and right to self-determination
- Builds collaborative relationships within and without of the agency to ensure the reputation of our agency and excellent care for our clients
- Maintains accurate and up-to-date records, reports and statistics as required by the agency
- Participates in staff meetings, agency committees and external committees as appropriate
- Other duties as assigned by the Executive Director or program supervisor.
Specific Responsibilities:
- Establish a trusting, therapeutic relationship with each client and their family
- Determine the support needs of each client
- Support clients during period prior to transition between care settings
- Work in collaboration with partners in care to prepare for the client’s entry into the destination environment
- Provide a comprehensive plan of care to the destination environment focused on minimizing the
- frequency and intensity of the risk behaviours
- Support staff, family members and care partners in the destination environment to implement the strategies outlined in the care plan
- Provide 3 – 6 weeks of hands-on support in destination environment
- Facilitate a gradual transfer of direct care through coaching and mentoring care providers in destination environment
- Provide support to assist client to successfully adjust to new living environment
- Collaborate with staff in new living environment to ensure that the client’s unique needs are met
- Ensure accurate record keeping of client and program data and statistics
- Provides input into program planning, development and evaluation
- Assist with training of new team members
- Participate in team meetings
- Other duties as assigned by the Executive Director or program supervisor.
