Our SISIP Rehabilitation Services Department, is looking for a self-motivated Registered Nurse to join our team as a Medical facilitator helping and advocating for our clients. Working from a home-based office you will be responsible for managing a client case load covering the geographical areas of Western Canada and Northern Ontario. The ideal candidate will reside in one of these regions and be responsible for regular liaison with clients. The incumbent will assist transitioning Canadian Armed Force Members with stabilization in the civilian world post release. This position is not a hands-on client care position; it is primarily administrative working autonomously from a home-based office. The ideal candidate will facilitate client access to supportive agencies and medical care. Often acting as a sound board the incumbent will guide clients to innovative solutions to facilitate their forward progress.

Special Conditions: This role is from a home-based office.

Developing, monitoring and implementing innovative and goal-oriented Nursing Plans.
Assisting with the transition process of ill and injured members of the Canadian Forces
Work with clients and shareholders to ensure client entitlements are clear and available.
Budget management for client support and home office
Provide effective communication to clients, policy holders and various medical professionals, often delivering complex and/or difficult messages.
Ensure the most effective and efficient supports are available to our clients.
Liaise with other support and partner agencies on behalf of the client.

Educational Qualifications:
Registered Nurse or Psychiatric Nurse,
Education with a focus in Psychology or psych experience would be an asset,
Counseling or community health nursing.
Registration in your appropriate certifying body.

Professional Qualifications:
Excellent written and oral communication skills in both French and English
Be good at timely note taking and maintaining legal documentation
Solid Word, Excel, Access and PowerPoint skills (Microsoft Office Suite)
Experience managing budgets
Experience with interacting with very difficult patients and community health.

Prior military experience would be a strong asset but not necessary
Ability to prioritize multi-task and prioritize when caseloads may be large and complex.
Excellent interpersonal and conflict management skills; the ability to find the win/win solution
Ability to make difficult decisions which may have significant impact on clients their families and our Business independently and deliver those decisions as required.
Proven problem solving skills.
Strength in working independently and in a cohesive team environment

Proven analytical, and organizational skills
Demonstrated ability to think of creative solutions to various challenges that frequently arise
