Position Summary:
The Responsive Behaviours (RB) Personal Support Worker (PSW) is part of an interdisciplinary team providing direct care, intervention, and support to older adults living in Fairmount Home (Fairmount); transitioning into or between healthcare and/or long-term care settings within the geographical boundaries of the South East Ontario Health atHome. The RB PSW will work with residents who are demonstrating responsive behaviours associated with the various types of dementia, delirium, mental health, addictions, or other neurological conditions that require behavioural support.
The RB PSW provides assistance with personal care and activities of daily living for residents of Fairmount Home. The RB PSW will work in collaboration with the in-home Responsive Behaviours team, Medical team, nursing team and PSWs as well as external care providers who specialize in mental health services to enhance the quality of life for residents. The RB PSW will participate in the assessment of responsive behaviours; provide direct care and support; identify triggers and successful strategies for responsive behaviours by assisting and modeling these strategies in collaboration with Long-Term Care Home (LTCH) staff. The RB PSW will carry out the duties in alignment to the organizations mission, vision and values, and work to further its priorities. The RB PSW will conduct themselves and their actions in accordance with the Fixing Long-Term Care Act, 2021.
Education and Experience:
- Post-secondary Personal Support Worker certificate from a recognized Community College and/or an accredited program as legislated in the FLTCA
- Equivalent combination of relevant education and experience as identified in the FLTCA may be considered including Graduate Nursing Assistant, Registered Practical Nurse, Graduated Nurse, Registered Nurse
- Recent and related experience with residents displaying responsive behaviours in a long-term care setting or other health-care setting
- Experience documenting and revising care plans in the electronic charting system
- Experience with quality improvement processes would be an asset
- Gentle Persuasion approach training required
- Experience leading others with the ability to clearly coach and guide coworkers in supporting residents with responsive behaviors
- Completion of relevant Behavioural Support Ontario 5 Star education modules including: DementiAbility, BSO foundations, U-First, and Gentle Persuasive approach or willingness and agreeance to obtaining this certification within 12 months of employment
