Position Summary
The Primary Care Registered Practical Nurse (RPN) provides comprehensive, patient-centered nursing care within an interdisciplinary primary care team. The RPN supports health promotion, disease prevention, chronic disease management, acute episodic care, and care coordination across the lifespan. Practice is guided by CNO standards, organizational policies, and evidence-informed clinical guidelines.

Key Responsibilities
1. Clinical Practice
- Conduct comprehensive health assessments including physical, psychosocial, and risk screening
- Develop, implement, and evaluate individualized care plans
- Provide chronic disease management (e.g., diabetes, hypertension, COPD, asthma)
- Deliver immunizations and preventative screening in accordance with public health guidelines
- Provide wound care, cryotherapy, suture/staple removal and other nursing interventions within scope of practice
- Identify and respond to clinical deterioration and emergencies
2. Health Promotion & Patient Education
- Provide patient education using health literacy and culturally responsive approaches
- Support self-management and behaviour change through motivational interviewing
- Address social determinants of health impacting patient outcomes
- Facilitate group education sessions as required
3. Interprofessional Collaboration
- Participate in team-based care within the Fort William Clinic
- Consult and collaborate with physicians and other team members regarding complex cases
- Participate in team meetings, case conferences and quality improvement initiatives
4. Quality Improvement & Accountability
- Participate in FHT quality improvement initiatives
- Support achievement of organizational performance indicators
- Contribute to policy and program development
- Maintain accurate, timely documentation in the electronic medical record (EMR)
- Ensure compliance with CNO standards, privacy legislation (PHIPA), and infection prevention protocols
Qualifications
Education & Certification
· Current Registered Practical Nurse (RPN) registration in good standing with the College of Nurses of Ontario
· Practical Nursing diploma from an accredited college
· Current BLS/CPR certification
Experience
- Minimum 2–3 years of clinical nursing experience; primary care experience preferred
- Experience working in team-based or community-based settings
- Experience in chronic disease management
Performance Expectations
All employees are expected to score at least 50% or higher on the identified Core Competencies and Key Responsibilities for their position. Scores with less than 50% may trigger a performance improvement plan and period of increased support and training.
Compensation & Benefits
· Base Salary – the salary for this position is determined by Ontario Health
· Extended Health Benefits – this position is not eligible for benefits and receives an additional 5.5% of pay in lieu of benefits
· Pension – this position is eligible for participation in HOOPP
