Job Summary

Responsibilities
- Deliver comprehensive patient care in various settings such as nursing homes, hospitals, assisted living facilities, and home care environments
- Administer medications accurately following physician orders and ensure proper documentation using electronic health record systems like Epic or Cerner
- Perform clinical procedures including phlebotomy, pain management interventions, and wound care
- Monitor patient vital signs, assess symptoms, and respond promptly to changes in condition
- Assist with activities of daily living for patients with dementia or hospice needs, ensuring dignity and comfort
- Collaborate with physicians, case managers, and other healthcare professionals to develop and implement individualized care plans
- Maintain detailed medical records and ensure compliance with healthcare regulations and safety standards
- Provide education to patients and their families on health management, medication use, and post-discharge care
Experience
- Proven experience working in nursing homes or hospital settings with a focus on acute care or senior care
- Hands-on experience in dementia care, hospice support, is highly desirable
- Skilled in medication administration, medical terminology, and laboratory procedures such as phlebotomy
- Familiarity with electronic health record systems including Epic and Cerner is preferred
- Background in home care or assisted living environments enhances your ability to deliver personalized patient-centered services
