Primary Care Registered Nurse Part Time Position (0.9 FTE)
Description:
Under the supervision of the Director, Medical Homes & Projects, the Primary Care Nurse provides comprehensive nursing services within the patient’s medical home. As part of a collaborative, inter-professional team, the nurse assists patients through all stages of life and various degrees of wellness and illness providing assessment, screening, healthy lifestyle support, education and chronic disease management, with the goal of improving patient health outcomes and facilitating access to services. The RN will be working at or out of more than one medical home, including the PCN central site. Part of the position will also be fulfilling the role of the Professional Practice Lead (PPL) for Nursing. The PPL is a leadership position that supports clinical development and professional growth for the PCN Nurse team.

QUALIFICATIONS:
Education and Certifications
- Baccalaureate degree in Nursing
- Current College of Registered Nurses of Alberta Registration
- Basic Cardiac Life Support Certification
- Must have own vehicle and valid driver’s license Experience
Experience
- Preference given to those with at least 5 years nursing experience
- Experience in the following areas is desirable: primary care, community-based care, chronic disease management, women’s health, and prenatal care
- Preference given to excellent leadership skills and project management experience
- Proficiency with electronic medical records is an asset
