The Transition Coordinator is a member of the LCFASD team, with specific direction in transition services for youth diagnosed with an FASD. Under the authority of the Executive Director, the Transition Coordinator assumes responsibility for carrying out assigned duties.
The Transition Coordinator will perform all duties while maintaining the integrity of the LCFASD, confidentiality and all other policies outlined in the Policy manual and Employee Handbook.
Duties and responsibilities Summary of Responsibility:
· Develop a process for families and youth with FASD to better manage transition planning to adult services
· Work with youth, families and agencies to develop a transition plan to adult services
· Apply policy and follow organizational procedures
· Gather statistical information for LCFASD’s data base
· Work within a team capacity at the LCFASD
Key Responsibilities:
· Determine the program opportunities in adult services for individuals with FASD
· Develop a comprehensive process for youth and families to transition to adult services in the least complicated manner
· Work with youth diagnosed with FASD to better understand their needs
· Work to establish relationships with youth and adult services to better meet the needs of young adults with FASD entering services.
· Review LCFASD files to determine statistical information on the number of youth needing services and contacting to see if they need assistance with transition.
· Maintain statistical information on youth
· Keep community agencies resourced with FASD information.
· Support agencies through the transition process
· Assist LCFASD in determining when youth clients should be transferred to an adult case manager
· Work with other agencies to learn what services are available in the LSA.
· Develop a good working knowledge of programming, educational opportunities, and services outside the LSA in order to assist families and LCFASD staff.
· Responsible to build strong working relationships/partnerships
· Communicate and work in collaboration with key stakeholders
· Provide education and training
· Bachelor Degree in Human Service field or equivalent
· Three years related experience, minimum
· Expertise in Fetal Alcohol Spectrum Disorder
The Transition Coordinator will work in the Lakeland area as designated by the LCFASD Board of Directors. The regional offices for the LCFASD are in Cold Lake, Bonnyville, St. Paul & Lac La Biche Alberta.
Hours of work are flexible within a one month time frame to a maximum of 37.25 hours per week and 149 hours in a 4 week period.

Pay: $47,000.00-$53,000.00 per year
