The Social Worker, Patient Capacity and Flow, is a member of Recovery Alberta's provincial Compass Integrated Operations Center that brings services and partners together with real time visibility and rapid escalation to improve flow, reduce delays and lower repeat hospital visits. This position will play a key role in supporting patient flow from inpatient settings to community housing programs. Working collaboratively with inpatient care teams, community partners, and housing providers, the Social Worker conducts comprehensive assessments, facilitates discharge planning, and coordinates housing placements for individuals with addiction and mental health needs. This specialized position requires extensive knowledge of community housing resources, supportive living options, concurrent disorders, and systemic barriers to housing. The incumbent serves as a liaison between inpatient and community services to promote seamless transitions, improve housing outcomes, and support timely discharge from hospital. Success in this role requires strong clinical assessment skills, systems knowledge, relationship-building abilities, creative problem-solving, and the ability to effectively manage competing priorities in a fast-paced environment. Some local travel may be required.
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:
Must have 3-5 years experience (within the last 7 years) in either a community or inpatient mental health setting Extensive knowledge of the housing sector Current CPR (BLS Level C – Health Care Provider) NCI - Non Violent Crisis Intervention A driver's license and access to a vehicle with appropriate business insurance Cultural competence Flexibility, openness, and a desire to be a strong team player
