Overview
We are a nurse-led home care company providing in-home care to seniors and people with disabilities. We are seeking compassionate and dedicated Health Care Aides to provide personalized care and essential support to clients in their homes.
The ideal candidate has a genuine passion for supporting others, strong caregiving skills, and a commitment to helping clients maintain their independence, dignity, and quality of life. This role offers the opportunity to build meaningful relationships while providing dependable, high-quality care.
Responsibilities
- Assist clients with activities of daily living, including bathing, dressing, grooming, toileting, and mobility support
- Prepare nutritious meals based on client preferences, dietary needs, and care plans
- Provide medication reminders and assistance according to the client’s care plan and established protocols
- Support clients living with dementia, chronic conditions, or disabilities using individualized approaches to care
- Perform light housekeeping and household tasks to maintain a clean, safe, and comfortable home environment
- Observe clients for changes in health, mobility, mood, behaviour, or overall well-being and report concerns promptly
- Complete accurate and timely documentation following each visit
- Provide companionship and emotional support while encouraging independence, choice, and dignity
- Communicate clearly and respectfully with clients, families, and the Ascent nursing team
- Follow individualized care plans and promptly report concerns or changes so our clinical team can respond appropriately
Qualifications
- Health Care Aide certification or equivalent education is preferred
- Current First Aid and CPR certification
- Ability to provide a satisfactory Criminal Record Check, including Vulnerable Sector Check
- Experience providing care to seniors or people living with disabilities, dementia, or chronic health conditions is an asset
- Previous experience in home care, supportive living, assisted living, or long-term care is an asset
- Knowledge of safe medication assistance practices and infection prevention procedures
- Ability to assist with personal care, meal preparation, light housekeeping, and mobility needs
- Strong interpersonal skills and the ability to communicate respectfully with clients, families, and healthcare professionals
- Strong verbal and written English communication skills
- Comfortable working independently in clients’ homes while remaining connected to the broader care team
- Valid driver’s licence and access to a reliable vehicle are considered assets
