Nurse Practitioner – GAIN – Permanent Full-Time

Position Overview
Our client is seeking an experienced Nurse Practitioner (NP) to join its GAIN (Geriatric Assessment and Intervention Network) team. This role provides advanced clinical care to frail and elderly patients in the community, with a focus on comprehensive assessment, diagnosis, treatment, care coordination, and collaboration with an interdisciplinary healthcare team.
The Nurse Practitioner will act as the Most Responsible Provider (MRP) for assigned patients, supported by collaborating physicians, and will provide care in accordance with the NP scope of practice, organizational policies, medical directives, and applicable standards.
Key Responsibilities
- Conduct comprehensive patient assessments, including health histories, interviews, physical examinations, and evaluation of psychosocial, cultural, and other factors affecting patient needs.
- Order and interpret appropriate diagnostic tests and procedures within the NP scope of practice, medical directives, protocols, and practice guidelines.
- Review health records and information from multiple sources to develop a comprehensive understanding of the patient's health status.
- Perform clinical assessments and establish medical diagnoses within the specialty scope of practice.
- Develop individualized goals, expected outcomes, and comprehensive plans of care in collaboration with patients and families.
- Prescribe medications, adjust dosages, and order treatments, interventions, and procedures within the NP scope of practice.
- Coordinate patient care and collaborate with physicians, allied health professionals, community services, and other members of the interdisciplinary team.
- Communicate care plans effectively to patients, families, caregivers, and healthcare professionals.
- Monitor patient responses to treatment and modify care plans as required to achieve optimal outcomes.
- Provide education and support to patients, families, caregivers, students, healthcare professionals, and other stakeholders as appropriate.
- Maintain accurate, timely, and comprehensive clinical documentation, including progress notes, referrals, forms, family conference notes, and team round documentation.
- Participate effectively in interdisciplinary team meetings and contribute to quality improvement and program development initiatives.
- Demonstrate leadership in advancing clinical practice and supporting the achievement of program goals.
Qualifications & Requirements
- Registered Nurse with an Extended Class Certificate (Nurse Practitioner) from the College of Nurses of Ontario.
- 3–5 years of recent, relevant clinical experience preferred.
- Current BCLS certification.
- Demonstrated knowledge of and ability to practice within the Ontario College of Nurses Standards of Practice for Nurse Practitioners.
- Valid Ontario driver's licence.
- Valid vehicle insurance with a minimum of $2 million liability coverage.
- PIECES and U-First training required.
Preferred Qualifications
- Master of Nursing degree.
- Gerontological Nursing Certificate (GNC(C)).
