The Scarborough Academic Family Health Team (SAFHT) is seeking a qualified and experienced individual to join our team in the role of Data Management Coordinator. This new role will report to the Executive Director and Manager and work in collaboration with our Quality Improvement Decision Support Specialist (QIDSS). The Data Management Coordinator will play an instrumental role in ensuring accurate and timely reporting to Ontario Health and other key stakeholders. This role will involve defining and coordinating reporting requirements, supporting the development of electronic medical record (EMR) queries for data collection, and conducting regular audits to ensure data accuracy and integrity.

The SAFHT is governed by a Mixed Board (i.e. Physicians and Community Members) and is an integrated group of health care providers that work collaboratively as part of a family health team. Our interprofessional care team currently includes 29 affiliated physicians, nurse practitioners, registered nurses, registered dietitians, a case manager, social workers, chiropodists, and several administrative staff. Originally formed in 2007, the SAFHT serves patients by providing them with timely access to a comprehensive range of services and programs aimed at managing and preventing disease and improving health status. Including our SAFHT affiliated physician group locations, we deliver care across 8 locations in the Scarborough and Eastern Toronto geography and engage in teaching learners in medicine and other health disciplines. Role Responsibilities:
● Works with the Executive Director, Manager, and QIDSS to coordinate the collection of data from SAFHT to fulfill the reporting requirements of Ontario Health.
● Data collection, analysis, measurement and reporting
● Support data collection strategies, evaluations, performance measurement, leverage data sources and benchmarking
● Develops KPIs, dashboards and reports for purposes of monitoring and optimizing organizational performance on quality, safety, operations, and planning for programs
● Plans, develops, designs, and implements reporting systems to support decision making needs
● Support EMR optimization and data extraction for reporting and improvement initiatives
● Develop, analyze, and maintain performance indicators, dashboards, and reports
● Support QIDSS with building EMR queries to collect measurement information.
● Support the preparation and presentation of detailed reports for meetings with key internal stakeholders and external stakeholders, as appropriate.
● Implement best practices for data management and auditing processes to identify and resolve any data quality issues. Respond to any queries from partners related to data reports.
● Participate in committees, working groups, and regional tables formed to develop data management best practices.
● Support Quarterly Reporting for Ontario Health involving EMR queries and completion of reporting template
● Providing support and training to SAFHT Interdisciplinary Healthcare Providers (IHPs) to optimize the efficient and effective use of the Telus PS Suites EMR and OceanMD platform, a secure EMR-integrated digital health platform that supports patient engagement and information exchange.
● Development and maintenance of EMR forms and other methods that help clinicians record patient information, manage workflows, and support clinical care.
● Manage EMR and OceanMD user access for SAFHT employees and provide orientation for new employees
● Assist SAFHT staff in collecting and interpreting data to support decision making relating to new and/or existing programs and services
● Build capacity among staff for data literacy and use of performance tools.
● Complete privacy audits at the direction of the SAFHT’s Privacy Officer
● Ensure data integrity, privacy, and compliance with relevant legislation (e.g., PHIPA)
● Will be required to support activities at multiple sites
● Ability to travel between multiple sites in the course of a work day
● Create data management systems and oversee data reporting
● Complete data analysis and make recommendations to the quality leadership
● Prepare regular reports and presentations
● Interpret and analyze trends in QI data for proactive identification of opportunities in the system
● This role will require you to work across our multiple sites, including our affiliated physician offices, and will also contain a hybrid component
● Additional duties as required
Specific Education and Experience Requirements:
● Undergraduate degree in a relevant discipline (Health Informatics, Data Management, Health information Management, Epidemiology, Information Technology, Health Sciences, Public Health, Quality, Biostatistics) or equivalent experience in healthcare data management, reporting, and database administration
● Two or more years of related experience, preferably in a healthcare/primary care setting
● Proficiency in Telus PS Suites EMR, data querying, and reporting tools is required
● Experience working with EMRs and healthcare data sets, particularly Telus PS and OceanMD
● 3-5 years of experience in healthcare analytics, decision support, quality improvement in primary care
● Experience in data quality and data governance
● Database management
● Spreadsheet and reporting tools
● Healthcare performance measurement and quality improvement
● Previous experience in handling confidential or sensitive information, including Personal Health Information (PHI)
● Valid Ontario Driver’s License and access to a reliable vehicle
