Andy’s House at Home is Hospice Muskoka’s community palliative care program that supports people in the last year of life, where they’re most comfortable — at home. Palliative care is covered under OHIP at no cost to patients. Through in-home visits and virtual care, our Nurse Practitioner-led team works alongside patients and families to create personalized Goals of Care that align with a palliative approach, and connecting them with additional Hospice Muskoka supports.
By working closely with local healthcare providers and supporting caregivers along the way, Andy’s House at Home helps people stay in their preferred setting, reduces the need for hospital visits, and ensures care is seamless, compassionate, and continuous.
Position Description:
The Nurse Consultant is responsible for providing comprehensive education and support to caregivers and family members of patients in the Community Hospice program. This role focuses on educating caregivers on palliative treatments, the patient’s disease progression, and pain and symptom management. The Nurse Consultant will work within the community, conducting educational visits in patient homes and facilities such as Retirement Homes and Long-Term Care Homes to ensure caregivers are equipped with the knowledge and skills necessary to provide quality palliative care.
Position Overview:
- Conduct home visits in collaboration with the Hospice Community Nurse Practitioner to educate caregivers and families on palliative care principles, disease progression, and pain and symptom management.
- Develop individualized education plans for caregivers that coordinate with the patients Hospice Care plan and is tailored to the patient’s diagnosis and care needs.
- Provide demonstrations of caregiving techniques, including medication administration, personal care, mobility assistance, identifying the need for pain and symptom management, and all components of their approved Hospice care plan.
- Assess caregivers’ understanding and competency, reinforcing learning as needed.
- Maintain accurate and confidential records of Community Hospice patients and document in accordance with PHIPPA and Hospice Muskoka policy.
- Develop and update Palliative Care educational materials for patients, families, caregivers, clinical staff, and volunteers.
- Ensure compliance with Hospice Palliative Care Ontario (HPCO) standards and Hospice Community Programs in relation to Palliative and end-of-life care.
- Serve as a resource for patients, families, caregivers and the Hospice care team on Palliative education-related issues.
- Clinical shifts in Andy’s House may be required from time to time to provide fulltime hours and cover off clinical needs.
Qualifications:
Education (degree/diploma/certifications) & Experience
- A completed college diploma (for RPNs) or university degree (for RNs) in nursing is required.
- Registered Nurse (RN) or Registered Practical Nurse (RPN) with a valid College of Nurses (CNO) license in good standing is required.
- LEAP Leaders course through Pallium Canada is considered an asset.
- A certified healthcare educator with hospice experience is considered an asset.
- A minimum of 3 years’ experience providing education to individuals in a clinical and non- clinical setting.
- A minimum of 3 years’ experience in hospice, palliative care, or a related healthcare setting.

- Empathetic, compassionate, and patient-centered approach, with a demonstrated ability to support individuals and families through complex emotional situations.
- Exceptional verbal and written communication skills, with the ability to give clear direction, document accurately, and represent Hospice Muskoka professionally.
- Highly organized and detail-oriented, with the ability to manage multiple tasks.
- Strict adherence to privacy and confidentiality regulations regarding Personal Health Information and Personal Information, in accordance with PHIPPA and organizational policy.
- A clear Criminal Records check with Vulnerable Sector Search is required.
- The following vaccinations are required: Measles, Mumps, Rubella, Varicella (chickenpox), Hepatitis B, Tetanus/Diphtheria/Pertussis (whooping cough), Influenza, and COVID-19.
- Two step Tuberculin testing is required.
Pay: $33.06-$45.53 per hour
