We currently have an opportunity for a Health Coach to join our Primary Health Clinic in the South Fraser Community located in Surrey and White Rock, B.C. In this role, you will support clients identified to be at risk for developing chronic disease, or in the early stages of chronic disease, by making realistic and maintainable lifestyle behavioural modifications.
Build on your education and experience as you:
Liaise with an interdisciplinary Primary Care Network team regarding referrals to determine behavior change opportunities such as those related to physical activity, nutrition, sleep, depressive and/or distressed symptoms.
Support clients with the co-creation and implementation of personalized wellness plans by utilizing approaches such as a stepped care approach focusing on improvements in lifestyle management behaviors and the setting of SMALL (specific, measurable, action-oriented, linked-to-your-life and long-term) goals.
Provide lifestyle coaching to clients based on the Small Changes health behavior change model, utilizing motivational interviewing techniques to address barriers.
Collaborate with and receive support and mentorship regarding coaching techniques from the registered psychologist.
Develop and facilitate group lifestyle coaching sessions for individuals with similar barriers as required and appropriate.
Here is what we are looking for you to have:
A Bachelor’s Degree in Psychology, Science in Food, Nutrition, and Health or other relevant health related field.
One (1) year of recent, related experience in providing lifestyle mentorship in community and primary care environments, or an equivalent combination of relevant education, training and experience.
Valid Class V B.C. Driver’s License and access to a personal vehicle for business related purposes.
This Relief Full Time position is available until November 30, 2022 or return of incumbent.
