Wetaskiwin Home Care is seeking an experienced Licensed Practical Nurse (LPN) to join our Home Care team. Reporting to the Care Manager and working collaboratively with the Case Manager RN, the successful candidate will coordinate and provide high-quality client care in the community. Using strong assessment, organizational, communication, and leadership skills, the LPN will work with clients, families, and the interdisciplinary team to ensure care services meet client needs. Responsibilities include assessing client needs, monitoring and adjusting care plans as required, communicating changes to the healthcare team, and providing direct nursing care when needed. The successful candidate will play a key role in delivering timely, client-centered care that supports clients to remain safely in their homes.
As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team, utilizing nursing processes, critical thinking, problem solving, and decision making skills. You will be guided by your education, experience, and demonstrated skill level, in alignment with applicable legislation, standards of practice, policies, and procedures. You will play a key role in providing safe, high quality, patient and family centred care while reflecting the shared vision and values of the organization.

Required Qualifications:
Completion of an accredited practical nurse education program. Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:
Access to a reliable vehicle for work-related travel. Driver's Abstract satisfactory to AHS and free of GDL restrictions. Commitment to Patient and Family Centred Care principles. Strong communication, interpersonal, and collaboration skills. Experience using Connect Care in a Home Care setting. Proficient computer skills. Competence in relevant nursing skills and procedures, with a commitment to ongoing learning and professional development such as experience in wound care, CVAD management, IV therapy, and other direct nursing care skills. Ability to work independently and as part of an interdisciplinary team.
Preferred Qualifications:
Minimum one (1) year of community nursing experience in Alberta. Demonstrated critical thinking, problem-solving, and conflict resolution skills. Strong time management and prioritization abilities in a fast-paced environment. Ability to work independently and collaboratively within an interdisciplinary team.
