Sunridge Healthcare Inc. is a private home care agency serving seniors and individuals across North York, Willowdale, Bayview Village, and the Greater Toronto Area. We serve Iranian/Persian, Korean, Filipino, Chinese, South Asian, and Italian communities. We are a growing agency that treats every PSW/ RPN as a valued professional not just a caregiver.
YOUR RESPONSIBILITIES
- Conduct initial home assessments for all new Sunridge Healthcare clients prior to commencement of care
- Develop individualized, written care plans in plain language accessible to PSW staff
- Reassess clients following any significant change in health status or care needs
- Review and update existing care plans on a periodic basis as required
- Provide clinical consultation to the Sunridge Healthcare coordinator as needed
- Communicate with client's family physician when clinically indicated
- Review incident reports and provide clinical guidance following any client incident
- Submit all assessment documentation within 48 hours of each visit
REQUIRED QUALIFICATIONS
- Active RPN registration in good standing with the College of Nurses of Ontario (CNO)
- Minimum 2 years of clinical experience in community, home care, or long-term care preferred
- Current CPR / BLS certification
- Valid Vulnerable Sector Check or willingness to obtain one
- Strong written communication skills ability to write clear, practical care plans for PSW staff
- Reliable, professional, and able to work independently with minimal supervision
STRONG ASSETS
- Fluency in Farsi/Persian, Korean, Tagalog, Mandarin, Cantonese, Hindi or Italian
- Experience in private home care or community health settings
- Experience writing care plans for PSW or support worker teams
- Valid Ontario G driver's licence and access to a vehicle
