Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role requires a strong foundation in nursing practices, excellent communication skills, and the ability to work effectively with diverse patient populations, including pediatrics, seniors, and individuals with dementia or hospice needs. The LPN will play a vital role in ensuring patient safety, administering medications, and supporting overall health management.
Responsibilities
- Deliver comprehensive patient care in accordance with physician orders and established protocols
- Administer medications and injections accurately while monitoring for adverse reactions
- Perform wound care, phlebotomy, and other clinical procedures as needed
- Assist with patient assessments, vital signs, and documentation using electronic health records such as Epic and Cerner
- Support patients with activities of daily living and provide emotional support
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Educate patients and families about health conditions, medication management, and post-discharge care instructions
- Ensure compliance with healthcare regulations and safety standards in all clinical activities
- Maintain accurate documentation of patient interactions and treatments
Experience
- Prior experience working in nursing homes, hospitals, or assisted living facilities is highly preferred
- Experience in acute care settings or home health environments is advantageous
- Familiarity with medical terminology, medication administration protocols, and laboratory procedures such as phlebotomy
- Knowledge of electronic health record systems including Epic and Cerner is desirable
- Background in dementia care, senior care, hospice care, pediatrics, or pain management is a plus
- Strong case management skills and experience working with diverse patient populations including children and elderly individuals are beneficial
- Proven ability to handle multiple responsibilities efficiently while maintaining compassionate patient interactions
