POSITION SUMMARY:
- Responsible for applying professional knowledge and competent practice in the provision of quality, resident-centered care based on the College of Nurses of Ontario and the Ministry of Health and Long-Term Care (MOHLTC) Long Term Care Homes Act (2007) (LTCHA) & Regulations, the Regional Values, policies and procedures and best practice guidelines.
- Provides guidance and leadership to the nursing team on the floor. Utilizes critical thinking and problem-solving processes and a customer service focus in the application of duties. Works in consultation with the Director of Nursing and Personal Care, Health and Wellness Director, Nurse Managers and Physician to assess the health needs of residents. Functions as a participating member of the Interdisciplinary Care Team.
MAJOR RESPONSIBILITIES:
- Clinical/Documentation
- Performs all duties under the direction of the Director of Nursing and Personal Care and Nurse Managers/RN, and in accordance with the Code of Conduct, and Saint Luke’s Place Mission, Vision and Values.
- Provides nursing care in accordance with the College of Nurses of Ontario Standards of Practice, administrative policies, and procedures of Saint Luke’s Place, and other applicable legislation.
- Communicates pertinent information to the health care team and always applies the Nursing Process in the provision of individualized care to residents. Documents all care and services provided in the resident’s health record, including but not limited to the resident’s care plan, progress notes, weights, and vitals etc.
- Responsible for the administration of medications and treatment as prescribed by the physician and in accordance with the College of Nurses of Ontario and for the documentation thereof.
- Responsible for the safekeeping of drug and narcotic keys for the unit to which they are assigned.
- Works collaboratively with the physician and the interdisciplinary care team on issues regarding residents’ health and carries out physician’s orders with follow up documentation, as required.
- Observes, assesses, manages, evaluates, records, and reports pertinent information regarding the resident’s condition to Physician, Director of Nursing and Personal Care and/or designate.
- Provides support and guidance to the Personal Support Workers for residents with complex/acute health issues and in emergency situations, including but not limited to chest pain, sudden shortness of breath, fall fracture, high fever, acute conditional state, delirium, acute responsive behaviour etc.
- Completes the resident assessments instrument (RAI) and Resident Assessment Protocols (RAPs) documentation on admission, quarterly as assigned and with significant changes in status.
- Utilizes the information gathered through the assessment and from the nursing team to develop the resident plan of care that is specific to the resident’s individual assessed needs.
- 11. Responsible for maintaining a current care plan that accurately identifies the specific needs, goals, and interventions for each resident.
- Leads the Interdisciplinary Restorative Care Program in collaboration with the RAI Specialist /designate as required.
- Responsible for the identification, referral, program implementation, assessment, monitoring quarterly and ongoing evaluation of program and documentation for residents on the Interdisciplinary Restorative Care program.
- Provides supervision and assistance in the dining room to ensure that all residents receive the appropriate diet and experience a quality dining service in accordance with Legislation and Home’s policies.
- Assists physicians and other external care providers on rounds and carries out instructions
- Provides health teaching to residents and family and acts as an advocate.
- Leadership
- Collaborates with the Director of Nursing and Personal Care in coordinating nursing assignments on the unit.
- Coaches, mentors, provides leadership, directs, monitors, evaluates, and follows up with the nursing team in the execution of their daily duties in adherence to the LTCHA, legislation, policies and procedures.
- Adheres to the MOHLTC LTCHA & Regulations to ensure compliance. Reviews PSW’s documentation for accuracy.
- Provides input for the performance evaluation of the Personal Support Workers and Nursing Attendants on the unit.
- Monitors and controls the usage of medical and personal care supplies. Notifies designate of when supplies are needed or may be required to order supplies.
- Orients new nursing staff and provides preceptorship to nursing students as directed.
- Communication/Customer Service
- Welcomes new residents/families and completes the admission process ensuring all relevant consents are obtained and documents signed.
- Prepares for, attends, and leads the interdisciplinary resident care conferences as required.
- Demonstrates customer service and effective communication to residents, families, staff, and all other internal and external contacts.
- Attends in-service education sessions, and workshops, as directed by the Director of Nursing and Personal Care/designate to maintain competency in long-term care.
- Attends and participates in staff meetings/unit team meetings and committees as required.
- Presents and assists in developing approved in-service education programs within the Home.
- Safety and Security
- Completes and analyses resident risk reports and implements corrective action as appropriate.
- Maintains a safe working environment by adhering to proper procedures and prompt reporting of unsafe conditions.
- Completes staff incident/accident reports and forwards to the Director of Nursing and Personal Care and/or Nurse Manager/RN, provides first aid to staff as applicable.
- Responsible for ensuring prompt response to nurse call and alarm systems.
- Participates in relevant aspects of the Continuous Quality Improvement (CQI) program to evaluate and improve resident services.
- Responds to and participates in emergency situations and emergency preparedness exercises as required.
- Reports health and safety hazards, malfunctioning equipment’s and actively participates in eliminating causes of accidents and performs emergency and occupational health and safety measures as required (WHMIS, Fire).
- Maintains infection control practices as per Provincial Infectious Diseases Advisory Committee (PIDAC) standards and directs the nursing team to do the same.
- Performs other delegated duties as assigned.
- LINE OF RESPONSIBILITY:
- Performs all delegated duties under the direction of the Nurse Manager/RN and the Director of Nursing and Personal Care.
EDUCATION, EXPERIENCE AND QUALIFICATIONS:
- Current certificate of competence as a Registered Practical Nurse with the College of Nurses of Ontario.
- Certificate in Gerontology is an asset.
- Experience in geriatric or long-term care, rehabilitation, medical/surgical, and psychiatric nursing is preferred.
- Strong leadership, interpersonal and teamwork abilities.
- Must have effective oral and written communication skills in English.
- Must be able to communicate appropriately with residents, staff, professionals, visitors, and volunteers.
- Must be able to work independently and within a team structure.
- Proficiency in basic computer skills is a requirement.
Must be physically able to perform all delegated duties.
