Cota is an accredited not-for-profit organization that has been providing community-based support to people living with mental health and cognitive challenges for over 50 years. At present, our wide array of services delivers much needed support to over 4,000 people/year who live with mental health challenges, psychogeriatric mental health conditions, acquired brain injuries, developmental disabilities, dual diagnoses and/or the experience of homelessness. About the Position
The Nurse Supervisor, Clinical Best Practice will provide onsite clinical oversight of Cota’s medication program, including participating in new tenant intake, discussion regarding medication changes, medication reconciliation, medication administration and rapid response to suspected or confirmed medication errors or other high-risk scenarios. They will also be the community-based lead for service user engagement, including the creation and implementation of a service user engagement strategy that maximizes engagement while aligning with Accreditation Canada best practices. The Nurse Supervisor will also operate as a part of the Performance, Quality & Risk Management leadership team, with direct involvement in client and staff safety initiatives, quality improvement activities, and accreditation readiness.
Primary Responsibilities:
The Nurse Supervisor, Clinical Best Practice’s primary duties may involve, but not necessarily be limited to:
Medication Program Oversight
- Conduct weekly onsite medication audits across all site-based supportive housing programs
- Liaise with pharmacies, prescribers, care teams, and external stakeholders
- Act as Co-chair for Cota’s Medication Committee
- Lead Cota’s accreditation preparation for the medication management standard
Incident Response
- Respond to medication incidents within the same or next day
- Lead investigations and implement corrective/preventative actions/resolutions in a timely manner
- Onsite outbreak response
Training and Education
- Provide clinical coaching, mentorship, and support to frontline staff
- Lead staff medication training in addition to facilitating / coaching for other trainings such as first aid and harm reduction
- Contribute to improve staff competency and confidence in the internal medication management support program
Service User Engagement
- Develop and implement a service user engagement strategy
- Support service user engagement initiatives and tenant participation activities
- Increase service user engagement at both site-based and community programs
- Support to broader community-based programs as needed (for service user engagement activities)
Other Responsibilities:
- Participate in accreditation preparation and quality improvement activities
- Collaborate on policy development and updates for portfolio
- Ensure timely and accurate documentation in electronic health records, including the eMAR
- Complies with all Health and Safety policies and procedures to ensure the protection of workers and service users and that health and safety concerns or incidents are responded to promptly
- Provides strong leadership as part of Cota’s Clinical Leadership Team and participates in Cota’s Leadership Forum and ad hoc committees as required
- Works cooperatively, respectfully and constructively with service users, team members, management, other service providers, and external stakeholders
- Ensures that confidentiality of service users, employees and Cota information is maintained at all times
- Undertakes other related and/or delegated duties, as assigned
Qualifications:
Knowledge and Abilities
- Strong knowledge of medication management and community mental health
- Experience in supportive housing, quality improvement, risk management, or clinical supervision
- Knowledge of IPAC standards, incident management, and accreditation processes
- Strong leadership, coaching, organizational, and crisis management skills
- Ability to collaborate effectively with interdisciplinary teams and external partners
- Commitment to client safety, recovery-oriented practice, and continuous quality improvement
- Strong organizational and time-management skills
- Ability to work independently and as part of a multidisciplinary healthcare team
- Clear written and verbal communication skills
Education and Experience
- Registered Nurse (RN) or Registered Practical Nurse (RPN)
