CASE MANAGER POSITION:
Salmo Community Resources (SCRS) is seeking a Case Manager for our Adult Mental Health and Substance Use (AMHSU) program. The Case Manager is a permanent part-time clinical position working with a complex client population of persons living with chronic and persistent mental illnesses, personality disorders and/or substance use issues in Salmo and surrounding area. The successful clinician will assess clients, develop treatment plans, provide brief intervention planning, work with a clinical and psychiatric team, utilize community resources, facilitate groups, evaluate outcomes, and fulfill contract-reporting requirements. Familiarity with electronic medical records is essential. This position is currently funded for 21 hours/week.
RESPONSIBILITIES:
The Case Manager:
· Implements care/treatment plans utilizing appropriate internal and external supports. Demonstrates appropriate clinical judgement in initial and ongoing assessment development.
· Provides assertive case management from a trauma-informed and culturally safe standpoint.
· Effectively teaches therapeutic interventions and provides psycho-education in a 1:1 or group setting. Facilitates therapeutic groups including relapse prevention groups. Provides counselling for individuals, families and groups.
· Provides crisis intervention and brief intervention planning when needed.
· Establishes and maintains clinical records in accordance with the funding body (Interior Health Authority).
· Communicates effectively with clients, families, and members of the SCRS and larger clinical team using verbal, written, computer communication means. Ability to effectively apply conflict resolution skills.
· Demonstrates capacity to effectively manage time and resources, establish priorities, organize work, and set objectives.
· Works as a team member within the SCRS clinical team and within Salmo to coordinate services for clients.
· Collaborates with other agencies throughout the region to deliver appropriate services for clients.
QUALIFICATIONS:
· A Bachelor’s or Master’s degree in social/behavioural sciences is required. Relevant degrees can include, but are not limited to, BSW or MSW (Clinical Specialization or equivalent training/education), MA/M.Ed (Counselling Psychology),or Graduate of a recognized school of nursing with current practicing registration with the College of Registered Nurses of BC (CBNBC) and/or the College of Registered Psychiatric Nurses of BC (CRPRNBC).
· Knowledge and skills in treating substance use and chronic mental health issues
· Minimum 3 years experience working with the target population
· Ability to assess, clinically evaluate, develop a treatment plan, utilize relevant resources in collaboration with the client/participant
· Competent with Meditech or other electronic medical record databases.
· Provides proof of vaccinations for Covid-19 when and as required by funder (Interior Health Authority).
· Excellent written and oral communication skills.
· Ability and desire to work as part of a team in a small, multi-faceted agency.

Interior Health Authority funds the position; thus, the successful clinician will be expected to work collaboratively with the medical and psychiatric community.
