Enhancing Care in the Community. Edmonton Zone Home Care is a health service that supports client wellness and independence. The goal is to help home care clients remain safe and independent in their own home or care setting for as long as possible. Alberta’s Home Care Program includes an array of services including health promotion and teaching, treatments, care at end of life, rehabilitation, home support and maintenance, assistance to maintain social connections, and support for the clients family or caregiver. The Home Care Program organizes health care service delivery with other health services that are available in the community. Within this advanced model of care, you will have the opportunity to collaborate with an interdisciplinary team of professionals providing support and care to clients of all ages. We are looking for an Occupational Therapist I - Case Manager to join our Southeast Home Care team. In this position, you will be involved in the case management of clients, including ongoing assessment, determination of unmet need, and care planning in consultation with clients to achieve client goals utilizing appropriate internal and external resources. Case Managers work within a Competency Framework that includes personal and professional accountability, systems thinking, creative thinking, communicating and building relationships. In addition, you will be responsible for the provision of direct Occupational Therapy services to clients on their case load including home safety assessment, equipment needs, cognitive screening, and functional assessment. As a Case Manager, you will provide necessary instruction and supervision to the Therapy Assistant to provide effective, efficient, and timely interventions to clients in their homes or clinic setting.
As an Occupational Therapist I, you will be working both independently and as part of an interdisciplinary team, responsible for enabling occupation through activities of assessment, treatment planning, intervention, consultation, education and follow-up, based on the patient's individualized goals, with outcomes focused on enabling the patient (client, resident) to perform meaningful daily occupations. You will provide clinical direction to therapy assistants, support personnel and students. You will review clinical referrals, accept, screen and assess client needs from an occupational therapy perspective in collaboration with other health disciplines. You will work in partnership with community agencies and providers as appropriate, to facilitate referrals and services to support care transitions in hospital and community settings. You will establish and maintain effective communication with colleagues, medical staff, and healthcare practitioners concerning documentation, treatment plans, progress and discharge. You will participate in client, family and caregiver education in collaboration with other disciplines, producing and maintaining clinical data, information, documentation and verbal reports to support the delivery of occupational therapy services, including charting.

Completion of master's degree in Occupational Therapy from an accredited university program or bachelor's degree in Occupational Therapy from an accredited university program and related experience. Active or eligible for registration and practice permit with the Alberta College of Occupational Therapists (ACOT).
Additional Required Qualifications:
Basic Cardiac Life Support- Healthcare Provider (BCLS-HCP). Proven communication and assessment skills to work cooperatively and effectively with the healthcare team members, clients, and their families in a fast paced environment. Strong professional judgment, initiative, and effective time management skills. Ability to work in different environments within the community (single family homes, lodges, walk up apartments, etc.) as well as lift and carry professional bags/equipment weighing up to approximately 25-30 lbs.
Experience in a community care setting and/or Home Care. Knowledge of Meditech. Case management experience.
