Bridgepoint Family Health Team provides primary care to a diverse patient population residing mainly in the East Toronto Sub-LHIN. Ours is a team-based, patient-centered model of primary care, with an emphasis on the patient being a key member of the care team. We have a dynamic interdisciplinary team that works closely to deliver high-quality primary care and exceptional patient experience. Our interdisciplinary team includes Physicians, Nurse Practitioner, Registered Nurses, Social Workers, Dietitians, Pharmacists, Medical Assistants and Administrative staff.
We are currently seeking a Permanent, Full-Time Nurse Practitioner for this exciting role. The Nurse Practitioner is a regulated member of the health care team who utilizes the nursing process, critical thinking, problem-solving and decision making skills to enhance care to individuals, families, groups and communities across the lifespan.
The Nurse Practitioner provides professional guidance to other health professionals and practices autonomously and interdependently within the context of an interdisciplinary health care team, making referrals to specialist physicians and others as appropriate.
Responsibilities include (but are not limited to):
- Provide holistic health assessments including health history and physical examination
- Work at full scope of the NP role, including the diagnosis, investigation and treatment of illnesses and injuries
- Screen for the presence of chronic illness and monitor/manage chronic illnesses
- ·Order appropriate diagnostic image, lab work and other investigations, and interpret results
- Determine the need for, and the implementation of health promotion and prevention strategies for families and individuals
- Routinely work with physicians and other members of the care team
- Participate at the organizational and administrative level as requested; e.g. be an active member of one or more committees, participate in any relevant program planning, assist in collecting statistics and/or generating reports, etc.
- Stay up-to-date with evidence-based best practices for Ontario NPs in primary care
The ideal candidate will have:
- Current registration from the College of Nurses of Ontario as an RN (EC);
- Completion of Master’s Degree in Nursing would be an asset;
- RNAO mental health certificate would be an asset;
- Minimum 2 years recent experience in a primary care setting;
- 1 to 2 years of experience providing mental health support and services;
- Previous experience in Case Management would be an asset;
- Effective patient/family counselling skills;
- Demonstrated ability for independent action, initiative, problem solving, organization and prioritization;
- Excellent communication, negotiation, teaching and interpersonal skills with the ability to work well with patients, families, staff and community;
- Experience in the development and maintenance of Medical Directives;
- Proficiency in the use of personal computer and software programs including Microsoft Office, email and internet. Experience in Electronic Medical Records would be an asset; Telus Practice Solutions preferable.

In addition to a competitive salary, this position also includes extended health benefits plan and enrollment in the HOOPP pension plan. Predictable working hours 9-5 pm Monday to Friday, No evening or weekend shifts.
