Calian is proud to continue a long-standing contractual relationship with the Department of National Defence (DND) to provide healthcare services to the Canadian Armed Forces. Through the Health Care Provider Contract, Calian employs health care professionals across Canada who are integral to supporting the health and wellbeing of our military members.
We are currently seeking a Registered Nurse (RN) Case Manager who would be interested in a full-time position at CFB Halifax.
The RN Case Manager is a key member of the primary health care delivery unit team and is responsible for providing case management services to CAF members who suffer from protracted and complex health issues. They facilitate access to health services based on the clients needs and programs and benefits for which they are eligible. They are directly involved in patient care coordination and conduct comprehensive case management assessments using the case management process with a goal of assisting patients return to active duty or to civilian life. The Case Manager works in collaboration with a multi-disciplinary team to ensure comprehensive care plans are developed for each patient.

- Bachelor’s Degree in Nursing
- Licensed in the Province of Nova Scotia
- Current CPR certification (for healthcare provider or level C)
- 3 years of full-time experience within the past 4 years performing assessments and developing care plans that reflect client’s individual needs as well as experience providing health education programs in a community health or primary care setting
Ability to obtain a Reliability Government Security Clearance.
