Simon Fraser Lodge is a modern complex care facility located in Prince George, BC and is home to 130 senior residents. We provide exceptional care & service in a home-like setting by focusing on quality, safety & accountability. Job Duties
In accordance with the established vision and values of the Facility and as a member of the Interdisciplinary Care & Service Team, the Social Worker is responsible for Social Services including psychosocial assessment, social histories, move-in and move-out planning, case management, counseling, advocacy, consultation, teaching and working with groups.
You also provide the following:
- Liaise with Family and outside Agencies
- Work closely with the Facility, the Health Authority and other groups/organizations as needed as part of the move-in and move-out process
- Manage financial issues upon admission and make necessary applications to Public Trustee, Social Services, HA Hardship waivers, DVA and other associations as required
- Provide counselling and education to clients and families
- Attend Care Conferences for all residents
- Chair Family Council and Ethics meetings
- Support the Care and Service Team through education as needed
- Make referrals to any necessary agencies and community resources
Qualifications:
- Registered Social Worker with the BC College of Social Workers- THIS IS REQUIRED- PLEASE DO NOT APPLY IF YOU DO NOT HAVE THIS DESIGNATION
- Two(2) years' recent related experience preferably in a long term care setting
- Experience in Health Care an asset
- Business License
- WCB Coverage
- Liability Insurance
Skills and Abilities:
- Demonstrated knowledge of Social Work casework techniques, including educational and group counselling
- Physical abilities to perform the duties of the position
- Demonstrated ability to work effectively (including stressful or emergency situations).
- Demonstrated ability to work effectively with others and within an interdisciplinary team
- Ability to communicate effectively verbally and in written documentation
- Ability to operate related equipment including related computer applications/software
- Ability to assess situations and defuse crisis incidents
- Familiarity with psychosocial rehabilitations and psychiatric disorders, medications and side effects
- Knowledge of relevant legislation
Pay: $38.60-$42.79 per hour
