Healing Hearts Foster Care Services is a mission-driven organization dedicated to advancing inclusion, safety, and well-being for children and youth living in foster homes. We are seeking a compassionate, skilled, and motivated Resource Worker to join our growing team.
As a Resource Worker, you will play a vital role in supporting children, youth, and foster parents through case management, advocacy, relationship-building, and day-to-day support. Your Role
- Develop a strong understanding of the children and youth supported and provide recommendations to enhance their daily functioning and well-being.
Case Management
- Conduct ongoing assessments and update plans of care as needs change.
- Establish short- and long-term goals using trauma-informed, strengths-based, youth-centered, anti-oppressive, and harm-reduction approaches.
- Develop discharge and transition plans, including crisis supports and community resources.
- Provide crisis intervention while modelling calm communication, problem-solving, and conflict resolution.
- Build professional, trusting relationships that support healthy social and emotional development.
On-Call Responsibilities
- Participate in an on-call rotation to provide after-hours support to foster parents, staff, and youth.
- Assess risk, provide guidance, and coordinate emergency, clinical, and community supports.
- Implement and support safety and crisis plans.
- Document all on-call activities in accordance with agency and Ministry requirements.
Support & Mentoring
- Provide guidance, referrals, advocacy, and skill development to children and youth.
- Act as liaison and advocate with families, schools, Children's Aid Societies, and community services.
- Coordinate and connect youth to additional supports as required.
Administration
- Complete assessments, service plans, and case documentation to MCCSS standards.
- Maintain confidential and accurate records in accordance with legal and ethical requirements.
- Participate in case management meetings and inter-agency planning.
- Support operational needs of the home when required (e.g., conflict resolution, reception, food programming, youth inquiries).
- Complete comprehensive, strengths-based assessments and contribute to individualized plans of care in collaboration with youth, foster parents, and support networks.
- Support children and youth to build independence at home and in the community.
- Promote healthy relationships with family, peers, community members, employers, and service providers.
- Work collaboratively with 1:1 staff and foster parents to establish consistent daily, weekly, and monthly routines.
- Provide encouragement, education, and coaching to build daily living and life skills.
- Support the physical, emotional, and medical needs of children and youth.
- Attend case conferences, team meetings, supervision, and agency meetings.
- Maintain accurate and timely case notes in ShareVision in accordance with agency and Ministry standards.
- Liaise with families, community partners, and professionals to coordinate care and services.
- Work both independently and as part of interdisciplinary teams while managing workload and deadlines effectively.
Your Qualifications
- Knowledge about and commitment to, the principles of inclusion, MCCSS (Ministry of Children, Community and Social Services), Foster Care, Children Aid Societies etc.
- A degree in Social Work, CYW and/or DSW and/or an equivalent combination of education/experience in the social services sector.
- 2+ years of experience providing case management to youth related to issues of homelessness, mental health, transitioning and/or coming out, substance use, family support, trauma, abuse, accessing legal/financial support, housing, employment, and education.
- 2+ years of experience in crisis support and intervention.
- Strong communication skills
- High energy level and desire for an exceptionally high level of stimulation, variety and change.
- Experience working with- Homebase and/or Sharevision
- Tolerance of, and ability to handle, a heavy workload and competing demands
- Current First Aid/CPR certification, UMAB, or willingness to obtain
- Valid driver’s license, acceptable driver’s abstract and reliable transportation to be used in course of employment
- Ability to obtain a clear Broad Record Check
- Ability, availability, and commitment to work a variety of shifts, including evenings, overnights, and weekends (if needed)
- Ability to manage conflict, crisis, and escalating situations calmly and confidently
- Commitment to social justice, harm reduction, anti-racist and anti-oppression practices, and to strengths-based, trauma-informed, participant-centered practices.

Pay: $45,000.00-$60,000.00 per year
