Job Description

The Edmonton North PCN is seeking a motivated individual to fill the role of a Mental Health Practitioner

The specific responsibilities of the Mental Health Practitionerinclude, but are not limited to:

  • Assessing the needs of referred patients over the telephone to determine relevant treatment plans which may include short-term counselling with a Mental Health Practitioner, and/or referral to group therapy, and/or a referral to an appropriate community agency or regional program
  • Counsel patients individually and in a group setting using a range of behavioral techniques such as motivational interviewing, readiness to change, behavioral intentions, self-monitoring, problem solving, goal setting, and others.
  • Educate patients and community members individually and/or in a group setting on the role of healthy lifestyle choices, active living and exercise in the prevention and management of a variety of chronic diseases based on principles of adult learning and effective instruction
  • Collaborate and consult with interdisciplinary team members as needed
  • Coordinate care with and/or link patients to other healthcare programs, healthcare providers or other relevant community agencies and resources as appropriate
  • Making professionally autonomous decisions in coordinating the care of patients
  • Ensuring the maintenance of accurate records with Electronic Medical Record, including the documentation and communication of a patient’s visits with the patient’s physician, completed in a timely manner
  • Administering metrics and screens to assess effectiveness of treatment
  • Providing mental health assessments, individual therapy based on comprehensive treatment plans, using knowledge of provisional DSM-5 diagnosis

Educational Requirements:

  • Master’s Degree in Social Work or Psychology; and/or Psychiatric Nurse or Registered Occupational Therapist
  • Active professional registration or registered and in good standing with a relevant College

Clinical Work Experience Requirements:

  • Experience conducting individual short-term counselling
  • Experience facilitating short-term group therapy
  • Strong assessment skills
  • Knowledge of community and regional programs

IMPORTANT TO NOTE:

Openings: Multiple, Full-time (Contract-Temp)

Requirementof working two evenings per week, 11:45 am – 8:00 pm.

Location: Northgate Centre (9499-137 Avenue)
