We are seeking a dedicated and knowledgeable Registered Dietitian to join our team.
Position Summary
The Dietitian provides person centered care to residents to promote the comfort, safety, and quality of life of the residents of Finlandia Nursing Home/Finlandia Village. As a key member of the Finlandia Village team and an integral part of an Interprofessional health care team, you will deliver nutritional care in accordance with Finlandia policies and professional practice standards. You will perform routine dietetic duties and ensure that the nutrition needs of the residents are met. Your primary role is to work in partnership with physicians and Interprofessional health care providers to perform resident nutrition interviews, assessments, and plans. You will deliver services to a variety of resident populations including high risk groups suffering from chronic diseases and/or complex multi-system diseases. You will also provide health education to residents with a focus on health promotion, disease prevention, and chronic disease management.
Some Key Responsibilities
- Maintains a working knowledge of the Fixing Long-Term Care Act and the Food Premises Regulation of the Health Protection and Promotion Act
- Monitors and revises individualized nutritional care plans based on assessment of resident’s current and changing dietary needs. Participates in care conferences and home area meetings as required
- Responsible for menu and production updates and distribution
- Resident nutrition reviews: monthly for high-risk residents or with wounds, quarterly for high-risk residents and annually with significant changes for all residents
- Addresses dietitian nutritional referrals and significant weight changes
- Admissions care plan development within 21 days of admission;
- For short stay residents who will be staying more than 14 days, completion of initial assessment & care plan within the stay period;
- Develops individualized meal plans
- Completes annual training & in-services as required
- Attends meeting, in person or by teleconference, with the Director of Dietary Services, family members, staff meetings, Ministry review of services, multidisciplinary care conferences, care plan meetings, or Pharmacy committee meetings as able;
- Attends resident care conferences
- Nutrition Analysis of menus and related reports as required. Nutritional analysis software/ analysis tools to be provided by Finlandia Nursing Home;
- Being available to answer questions for Ministry of Health when assessments/audits are being conducted for Finlandia Village
- Develops and maintains a thorough working knowledge of the Finlandia Village Health and Safety Manual and associated Provincial Legislation. Performs Health and Safety inspections and investigations as required
- Develops and maintains a thorough working knowledge of Finlandia’s Resident Safety Policies.
- Performs other duties as assigned
Qualifications
- University degree in Dietetics and a dietetic internship as recognized by the College of Dietitians of Ontario
- 1-2 years of experience in long term care setting an asset
- Member in good standing with the College of Dietitians of Ontario
- Must possess strong written and oral communication, critical thinking, care coordination, time management and customer focus skills
- Must have proficient computer skills using Microsoft Office applications (Microsoft Word, Excel, PowerPoint), Point Click Care/POC/eMAR
- Finnish speaking skill is considered an asset
- An acceptable Police Vulnerable Sector Check verified within the past six months
- 2-step TB test within the past six months
