Patient Transfer Coordinator (PTC), BCEHS (Casual)

Help us safely and efficiently transfer patients from paramedic to hospital care, so we can provide the best healthcare possible.
In this role, you will:
Work with a team in contact centre environment
Receive telephone calls or online requests
Enter patient and case information into a database
Arrange conference calls between the referring and receiving physicians/care provider/ delegate when required
Monitor transport status and coordinate low acuity transfer plans
Help maintain an accurate bed availability database and produce reports
We are recruiting multiple Patient Transfer Coordinators. The start date is to be determined. The required training period is 12 to 13 weeks at approximately full-time capacity. You must have flexibility to work shift work (up to 12 hours) including nights, weekends and statutory holidays in a 24/7 contact centre environment.
As one of our successful candidates, you will:
Receive patient transfer requests by telephone or online, collects information, and triage cases to the appropriate case management team
Handle information requests or directs to appropriate team
Process patient transfer requests in the database and follow on-line process maps and resources and search for available beds
Support the Clinical Transfer Nurse (CTN) with high acuity patient transfers
Coordinate low acuity transfer plans
Commission transport providers, confirming and monitoring patient transfer plans and escalating cases that deviate from initial transfer plan to team CTN. Determine the most appropriate transport provider for non-medical transfers.
Work with facilities to monitor the status of transferred patients to ensure the transfer system can support patient discharge plans.
Help patients return safely to their home communities by providing advice and guidance to facilities
Help maintain an accurate availability database and pull standardized reports
Identify problems and/or inconsistencies, as well as opportunities for improvement and provide feedback and recommendations to PTC Supervisor
Enter data, complete forms and forwards files
Performs other duties as assigned
A high school diploma
A basic understanding of medical terminology
Graduated from a recognized Nursing Unit Clerk program
Three (3) years' recent related experience or an equivalent combination of education, training and experience
Communicate effectively verbally and in writing
Work in a collaborative team environment
Keyboard at least 50 w.p.m.
Organize and prioritize work tasks
Operate related equipment
Use computer software applications
Physical ability to carry out the duties
Qualified applicants will be invited for a 2.5 hour assessment session which, if successful, will be followed by an interview.The applicants will be tested on:
Call Centre Etiquette
CritiCall (computer simulation testing)
Applicants who are successful in the Assessment Day will be contacted to for an interview. Candidates who receive a job offer will need to obtain a satisfactory Criminal Record Check.
