At Oasis Fertility Centre, we support individuals and couples on their fertility journey with compassion, education, and high-quality care. Our team ensures every patient has the guidance and resources they need throughout their treatment.
We are looking for a Licensed Practical Nurse (LPN) to join us as a Patient Care Coordinator. In this role, you will use your clinical background to guide patients from their initial consultation through treatment completion. You will act as the main point of contact for patient communication, scheduling, and care management. You will also be trained in clinical procedures to provide hands-on nursing support when needed.
Key Responsibilities
- Patient Coordination: Act as the primary contact for patients. Answer questions, explain treatment plans and timelines, and guide patients through each step of their care. Coordinate details across physicians, lab staff, and nursing teams.
- Scheduling & Administration: Manage patient appointments and update the clinic calendar, including the Medical Director’s schedule. Onboard new patients into the clinic database, process payments, and assist with insurance documentation.
- Clinical Duties: Complete training in clinical components to assist with direct patient care, phlebotomy, and clinic procedures as required.
- Documentation & Compliance: Maintain accurate records in Netcare and internal EMR systems. Follow clinic policies, regulatory standards, and health privacy requirements.
- Quality & Operations: Participate in team meetings, track daily clinic metrics, and assist with process improvement initiatives to support patient care standards.
Requirements
- Active LPN registration in good standing with the CLPNA.
- Clinical experience in a healthcare setting; fertility or women’s health experience is an asset.
- Phlebotomy certification, current BLS and ACLS certifications, and up-to-date immunizations.
- Strong organizational skills and attention to detail.
- Clear written and verbal communication skills.
- Experience with EMR systems and Netcare preferred.
- Ability to work well independently and alongside a multidisciplinary team.
Pay: $26.00-$32.00 per hour
