The Attending Nurse Practitioner (NP) reports directly to the Senior Nursing Manager or Administrator and is also accountable to the Medical Director for meeting the long-term care home’s (LTCH) policies, procedures, and protocols for medical services. The Attending NP is a primary care provider to residents and works within their legislative scope of practice as described and outlined by the College of Nurses of Ontario. The Attending NP collaborates with the resident or substitute decision maker (SDM), and the health care team in the development, implementation and evaluation of the resident’s plan of care. The Attending NP will also provide leadership and mentorship to LTCH staff that enhances staffs’ knowledge, assessment skills, and ability to care for residents in place, and leads and collaborates in research, education, and evidence-based practice initiatives to optimize resident, LTCH and health system outcomes.
- Provide comprehensive primary care to residents as part of the interdisciplinary health care team.
- Conduct person-centered comprehensive health histories as required.
- Collaborate with the resident to develop a person-centered plan of care.
- Utilize communication and counselling skills by engaging residents in meaningful dialogue to determine what is important to them for health and quality of life.
- Provide person-focused health education.
- Participate in care conferences on an as needed basis.
- Manage the care of residents by providing pharmacological, complementary and/or counselling interventions, and perform procedures within the NP scope of practice.
- Order and/or perform appropriate screening and diagnostic investigations, and interpret results within the NP scope of practice.
- Diagnose acute and chronic health conditions within the NP scope of practice.
- Participate in providing after hours coverage of the home for clinically complex resident consultation.
- Increase and facilitate continuity of care through collaboration, consultation and referral as appropriate.
- Collaborate and consult with physicians, the Senior Nursing Manager, nursing staff, interdisciplinary team members and external healthcare partners and stakeholders regarding a resident’s plan of care.
- Assist, support, guide and provide consultation to the Senior Nursing Manager, nursing staff and interdisciplinary team members regarding complex clinical situations.
- Make referrals to specialized consultants, services, and other health care providers.
- Advocate for and provide palliative and end-of-life care.
- Document clinical data, assessment findings, diagnoses, plans of care, therapeutic interventions, resident responses, and clinical rationale in a timely and accurate manner.
- Participate in creating an organizational environment that supports the safety quality of resident care and life, collaborative practice, and professional growth.
- Identify, develop and participate in the implementation of practice innovations, in collaboration with the LTCH’s senior leadership team and Shared Services.
- Participate in the implementation of strategies to optimize the integration of illness and injury prevention, health promotion, health maintenance, rehabilitation and restorative care activities.
- Participate with the senior leadership team in program planning to meet the needs of short and long-term residents and of residents of varying age groups.
- Participate in care conferences as required.
Essential
- Registration in the Extended Class as a Nurse Practitioner.
- Registration with the College of Nurses of Ontario.
- Bachelor of Nursing from a recognized University.
- Ability to communicate effectively with internal and external groups to accomplish client specific and organizational goals and objectives, excellent writing and communication skills, skill in critical appraisal of research studies, knowledge of health promotion, and demonstrated excellence in leadership in a team setting.
- Proficient with computers and a variety of software.
Preferred
- Three years of recent experience in a long-term care or public/community health setting, a minimum of two years’ experience in a long term care nursing role.
Working Conditions
- Travel will be required between locations within Halton Region. Incumbent must supply own transportation.
- Flexible work schedule is required to respond to the needs of the home.
- Position includes 4 weeks paid vacation.
Employment Conditions
- Current (obtained within the past six (6) months), original and acceptable Criminal Records Check, including Vulnerable Sector Screening by the first day of employment.
- Current two-step tuberculosis test (obtained within the past six (6) months) by the first day of employment.
