Critical Trauma Therapy has an exciting opportunity for a compassionate and client-centered Nurse Practitioner to join our team in a hybrid role of virtual and direct care (depending on geographical locations). The NP will provide comprehensive patient care, including assessments, treatment plans, and follow-up care. The ideal candidate will have experience in working with individuals diagnosed with: Traumatic Brain Injury, chronic pain, mental health concerns, and/or ADHD. The ideal candidate will also have paediatric experience.
Key Responsibilities
- Deliver holistic and collaborative primary care within the full scope of practice outlined by the College of Nurses of Ontario.
- Support individuals recovering from motor vehicle collisions, primarily those managing chronic pain, traumatic brain injuries, musculoskeletal conditions, mental health challenges, ASD and ADHD.
- Conduct thorough assessments and provide evidence-based treatment for mental health, ASD and ADHD through a trauma-informed approach.
- Prepare and submit requisitions for diagnostic imaging (MRI, CT, bloodwork) and laboratory tests, ensuring compliance with protocols and timely follow-up on results.
- Design, implement, and monitor individualized care plans tailored to each patient’s needs and aligned with best practice guidelines.
- Maintain comprehensive, accurate, and up-to-date medical documentation in accordance with legal, ethical, and organizational standards.
- Complete and submit necessary medical and insurance documentation, including Disability Tax Credit forms, ODSP applications, and auto insurance reports.
- Lead or contribute to the development and delivery of targeted health programs, such as postpartum women’s wellness initiatives.
- Review of treatment plans and medical files to identify gaps in service delivery and create a plan of action
*** The successful applicant for the Oakville Trafalgar Memorial Site will be trained on the use of deepTMS for mental health conditions.
Qualifications
· Master’s degree in Nursing from an accredited program.
· Current registration as a Nurse Practitioner with the College of Nurses of Ontario.
· Minimum of 2 years of clinical experience in a primary care or relevant healthcare setting.
· MVA sector experience is considered a strong asset.
· Excellent communication and interpersonal skills.
· Ability to work independently and as part of a collaborative team.
