Nurse Practitioner
Full-Time Contract Position, For a Period of up to 13-months
This position provides an exciting opportunity join a dynamic team of local physicians and interdisciplinary health care professionals at our Paisley and Walkerton Sites.
Main Responsibilities:
- Work collaboratively with physicians and inter-disciplinary team members to effectively manage patient needs.
- Provide a full range of primary care services including assessment, diagnosis, treatment, follow-up, education, consultation, and referral.
- Deliver health promotion and disease prevention management and support.
Position Qualifications and Preferred Experience:
- Bachelor’s degree in Nursing.
- Current Certificate of Registration in the Extended Class from the College of Nurses of Ontario, and eligibility to practice as a RN-EC.
- Experience practicing as a NP in a Community Health or Primary Care setting.
- Proven ability to communicate effectively with patients, families, and members of an interdisciplinary team.
- Experience in delivering patient education and chronic disease management.
- Proficiency in the use of computers and healthcare information technology.
- Possession of a valid driver’s license and access to a reliable vehicle.
