Intake Coordinator Admission and Assessment

The Care and Community Branch of Continuing Care supports multiple programs including the intake and assessment for both home care services and Continuing Care facilities. We are looking for a Registered Nurse that has a knowledge of Continuing Care, as well as intake and assessment processes. This position reports to the Director, Care and Community and is responsible for the intake and referrals for Continuing Care programs, completion of standardized assessments, evaluation of care requirements, and assignment of client files for long and short term care services.

Essential Qualifications:
The ideal candidate meets the following qualifications; specifically, how your work experiences and related duties align with the essential qualifications shown below.

Eligible for registration as Registered Nurse or Nurse Practitioner with Yukon Registered Nurses Association
Experience working as an Register Nurse in a community setting
Experience in assessing and evaluating care requirements for clients, including use of standardized assessment
Experience with utilizing electronic client records such as Goldcare, Meditech, Plexia, etc.

Desired Knowledge, Skills and Suitability:
Candidates should have and may be assessed on:
Strong ability to collaborate and work within a multi-disciplinary team and with other health professionals, community and government agencies.
Knowledge of intake and assessment tools and procedures
Excellent clinical judgement and decision-making skills when assessing information and needs of clients

Additional Information:
Conditions of Employment: Current registration with the Yukon Registered Nurses Association, TB Screening, CPR Level C within 18 months, Transportation of Dangerous Goods Certificate, Valid Class 5 Drivers Licence without daylight restrictions, WHIMIS 2015, Vulnerable Sector RCMP Security Clearance

Job Requirements: Lifting up to 20 lbs, travel within the Territory, exposure to pets, second hand smoke, plants, flowers, and latex products.
