OVERVIEW
The Manager of Outreach and Integrations will play a critical role in ensuring the seamless implementation of new processes, systems, and practices to further integration between hospital and community in the context of patient transitions and supporting outreach to vulnerable populations that access care at MGH. This position requires strong leadership skills and the ability to manage multiple projects simultaneously. The Manager will report to the Director of Integrated Care, providing operational oversight and executing key projects to enhance integrated care pathways and improving patient outcomes.
The Manager will oversee a variety of teams engaged in outreach and integration work, ensuring accountability and performance of said teams. . The Manager will oversee staff in the East Toronto Primary Care Navigation Clinic (ET-PCNC), Nursing Led Outreach Teach (NLOT) and the Integrated Neighbourhood Home Care Program (INHCP). The manager is responsible for the hiring, support, performance management of said teams as well as working with external program partners for the programs to ensure efficient operations. The manager supports team members and works with community partners to coordinate and integrate services and will focus on on-going performance improvement. The incumbent supports the team by actively participating in projects to support transitions to community settings such as patient homes and long-term care homes and identifying and implementing new initiatives to support transitions for patients that are vulnerable for readmission upon discharge with a goal of reducing non acute LOS, reducing readmission rates and reducing overall ALC rates.
The incumbent contributes to corporate initiatives and quality improvement projects related to ALC, home care modernization and patient flow with a particular focus on reducing non-acute length of stay (LOS), hospital readmission rates and rates of unattached patients in hospital. As such, the manager will work closely with Managers of Transitions and Integrations and Patient Flow, supporting system flow.
EDUCATION
- Baccalaureate Degree Required.
- Masters in a related field preferred.
- Current registration with Health Professional College / Association required.
- Demonstrated commitment to on-going learning and professional development required.
QUALIFICATIONS
- Demonstrated experience in incremental leadership responsibilities in Hospital Operations.
- Familiarity with improvement process principles is a definite asset.
- Experience with community service, home care and/or primary care an asset.
- Demonstrates a process driven approach to problem solving.
- Demonstrated computer literacy with competence in Microsoft Word, Excel, PowerPoint, Outlook, Powerchart and Internet Explorer.
- Flexible, organized and self-directed within scope of responsibility; takes initiative.
- Knowledge of process improvement methodologies and quality monitoring required.
- Excellent written and oral communication skills.
- Exceptional interpersonal skills.
- Demonstrated ability to multi-task, problem-solve with proven critical and creative thinking skills.- Evidence of ongoing education (e.g. Workshops, seminars, etc.) and up-grading of technical skills and knowledge.
- Demonstrated ability to manage conflicting priorities.
- Demonstrated ability to collaborate and develop relationships with internal and external partners.
