GERIATRIC ASSESSMENT CLINICIAN
Registered Nurse or Pharmacist
Full Time Permanent 1.0 FTE (37.5 hours/week)
Reporting to the Clinical Manager, Clinical Director or designate, the Geriatric Assessment Clinician supports the delivery of Primary Care Services in the family practice clinic setting. The Geriatric Assessment Clinician works collaboratively with Edmonton O-day’min PCN (Primary Care Network) family physicians and other interdisciplinary team members to implement primary care network initiatives for patients with the Senior’s Hub guidelines and setting.
KEY RESPONSIBILITIES & DUTIES
The Geriatric Assessment Clinician provides optimal primary care for frail geriatric patients with chronic disease or complex care needs including:
- Screening, assessment, and monitoring of frail geriatric patients
- Coordinating and collaborating ambulatory services with other Geriatric Hub team members
- Managing and following-up with patient’s links to community services
- Providing evidence-based interventions and education to patients and their families regarding geriatric health issues, medications and treatment options using developed educational materials
- Liaising and care coordination with both PCN team members (e.g., mental health coordinators, kinesiologists, nurses, psychologists, and pharmacists) and outside agencies
- Maintain accurate and up-to-date documentation of assessments, interventions, and outcomes in electronic health records.
QUALIFICATIONS:
Minimum of 3 years of Geriatric Nursing experience within a complex primary care or community setting required.
- Registered Nurses (BScN) or Registered Pharmacists required Current registration with relevant college (CRNA or Alberta College of Pharmacy)
- Current CPR (Heart and Stroke Foundation BLS Level C- Health Care Provider).
- Demonstrated proficiency in computer software such as electronic medical records, Microsoft Office (including PowerPoint)
- Excellent interpersonal, collaborative and team skills
- Clear communication, critical thinking, and organizational skills
- Clear criminal record check with a vulnerable sector check
- Valid driver's license as some travel is required
