Overview
We are seeking a dedicated and experienced Licensed Practical Nurse (LPN) or Registered Nurse (RN) Consultant to support our growing home care team. This role focuses on caregiver training, client assessments, care plan development, and clinical consultation to ensure our clients receive safe, compassionate, and high-quality care in their homes. The ideal candidate will have experience working with seniors, strong clinical judgment, and a passion for supporting both clients and caregivers.
Duties
- Conduct client assessments and identify care needs, risks, and appropriate service recommendations
- Develop, review, and update individualized care plans in collaboration with clients, families, and healthcare providers
- Train and mentor Health Care Aides (HCAs) and caregivers on best practices for home care
- Provide education on topics such as dementia care, personal care, infection prevention, fall prevention, safe transfers, and documentation standards
- Perform caregiver competency evaluations and provide ongoing coaching and support
- Review client concerns, incidents, and changes in condition, providing recommendations as needed
- Conduct quality assurance audits of care documentation and service delivery
- Assist with the development and maintenance of company policies, procedures, and clinical standards
- Serve as a clinical resource for caregivers, management, clients, and families
- Support communication and collaboration with physicians, therapists, community healthcare providers, and family members when appropriate
- Maintain accurate and professional documentation in accordance with company policies and applicable regulations
Skills
- Current LPN or RN registration in good standing with the appropriate British Columbia regulatory college
- Experience working with seniors, home care, community care, long-term care, or related healthcare settings
- Strong knowledge of patient care, nursing principles, medical terminology, and healthcare best practices
- Experience training, mentoring, or supervising caregivers is considered an asset
- Excellent assessment, problem-solving, and clinical decision-making skills
- Strong communication and interpersonal skills with the ability to educate and support clients, families, and staff
- Compassionate, professional, and client-centered approach to care
- Ability to work independently and effectively manage multiple priorities
- Strong organizational and documentation skills
- Valid BC driver's license and reliable transportation

Pay: $36.38-$42.00 per hour
