The CCRS Coordinator is a Toronto Rehab clinical lead for the Continuing Care Reporting System (CCRS). The CCRS (RAI) Coordinator/Educator is responsible to support all aspects of Continuing Care Reporting System (CCRS) data collection, audit, and reporting as well as participating in various quality improvement initiatives with an aim to improve outcomes and funding opportunities. The Coordinator works collaboratively with the clinical staff, Managers, Decision Support, SIMs and Finance to ensure timely and accurate entry of data, data quality and timely submissions to CIHI and MOHLTC. The role supports analysis of outcomes and case costing, impacting patient care and hospital funding.
Bachelor’s Degree in a regulated health profession (Masters level preferred)
Registered and in good standing with respective regulatory College
2 years relevant clinical experience, ideally in CCC, LTC or rehabilitation environment
A minimum of 1 year experience related to CCRS (RAI-MDS) assessment, training and implementation preferred
Certified in Coding the RAI-MDS (Experienced Assessors) preferred
Knowledge of health care reporting requirements and CCRS Case mix systems preferred
Self-motivated, ability to work independently and as a member of a team
Excellent analytical skills with great attention to detail
Advanced computer proficiency (Microsoft Word, Excel, PowerPoint)
Exceptional communication (written and verbal), interpersonal skills (ability to develop a rapport with staff) and organizational skills (Ability to prioritize work)
Knowledge of adult education principles and demonstrated teaching/facilitation skills
Evidence of good performance history and attendance record
Ability to work in a manner that exemplifies the core values of University Health Network
Capable of interpreting the data and reports to identify trends, issues and opportunities.
Project management experience
CCRS Data Quality Support:
Work with teams on the units to improve processes to ensure timely, accurate and efficient completion of CCRS assessments
Coordinate complete of assessment
Complete CCRS Assessments as necessary
Oversee data quality checks (audits) for CCRS assessments and perform chart reviews/audits as necessary to review accuracy and consistency of coding and assessment information .
Ensure CCRS data quality by monitoring CCRS activities on the units and conducting data quality audits.
Support inter-professional teams with strategies to complete CCRS assessments as per required standards.
Liaise with Service Coordinators/Admitting Representatives/Program Assistants, Clinical Staff, Decision Support, and Patient Service Managers regarding CCRS data quality issues
Review reports including outcomes, case mix and comparisons to peers to identify opportunities for improvement and implications on patient care and/funding
Function as a resource for clinical team members regarding CCRS assessments, RUG Weighted grouping, incomplete assessments, missing data, data errors etc.
Assist teams with CCRS coding challenges, liaising with CIHI as necessary for clarification or resolution
Function as a resource and link with Decision Support and IT staff regarding CCRS software interface issues, monthly/quarterly submission reports, annual software upgrades etc.
Remain abreast of and communicate changes to reporting requirements, modify processes and implement as necessary
Liaise with vendors as necessary when support is required
Represent UHN at external committees in relation to CCRS
Oversee the education, training and CCRS certification/re-certification process for clinical staff.
Ensure staff appreciates importance of accurate CCRS data entry
Oversee the education and training of administrative and clinical staff with the CCRS assessment software.
Maintain and revise CCRS educational/assessment materials based on CIHI and/or software vendor updates.
Provide education and support in the use of the clinical data
