Build on your career experience by:
Working in collaboration with the Patient Care Coordinator and the interprofessional team with facilitating patients' hospitalization from admission through discharge
Coordinating and collaborating with the interprofessional team to expedite effective, quality patient care and coordinates timely and sustainable discharge plans
Intervening to address barriers and providing leadership as needed to achieve optimal outcomes including helping to expedite additional services as required
Being accountable for the development and effectiveness of policies, procedures and standards to support effective care management and discharge planning
Evaluating progress towards desired outcomes to ensure plan of care and services provided are patient-focused, of high quality, efficient and effective
Your background includes:
Three (3) years recent, related acute care experience including one (1) year of experience in community care and discharge planning
Current practicing registration as a Registered Nurse with the British Columbia College of Nursing Professionals (BCCNP)
This role is a casual position located at ARHCC in Abbotsford, BC. ARHCC is a 272 bed, University of British Columbia affiliated hospital offering medical, surgical, obstetrics, inpatient pediatrics, renal, cardiology, tertiary palliative care, helipad, as well as 24/7 emergency services with 79,289 visits annually.
