As a Patient Care Coordinator t at Naz Pharmacy, you will play a key role in supporting pharmacy operations to ensure the highest level of service to our patients and the smooth functioning of our specialty programs, particularly Home Monitoring. This position encompasses both technical and administrative tasks and requires collaboration with various stakeholders to maintain efficiency and patient satisfaction.
Duties and Responsibilities
Your primary responsibilities include, but are not limited to:
Act as the main communicator with the Home Monitoring (HM) team at the head office.
Contact case managers and coordinate with relevant parties for patient care and route management.
Assist with the intake and assessment of patient referrals for the Home Monitoring program and other programs.
Collaborate with home monitoring pharmacies to streamline processes that support both the team and program.
Support the office team with technical duties and ad hoc requests as needed.
Manage sick calls and coordinate coverage; step in to provide relief coverage when alternative solutions are unavailable.
Oversee the management of company vehicles, including scheduling repairs, services, and maintaining a weekly car assignment schedule to assist the nursing team.
Assist pharmacists in maintaining medication inventories and accurate prescription records.
Plan and communicate daily deliverables with pharmacists and the Pharmacy Manager to ensure alignment with pharmacy objectives.
Demonstrate a proactive approach in identifying areas for efficiency improvements.

Key Requirements
Some experience or education in a pharmacy assistant or similar role.
Physical ability to stand for the duration of shifts and lift up to 15 kg (35 lbs).
Proficiency with CRMs.
Assist with typing and processing home monitoring prescriptions.
Support tasks related to the pharmacy calendar and broader Home Monitoring needs
