Position Summary :
The Registered Dietitian is an active and essential member of the interdisciplinary Collaborative Practice Team. The incumbent is responsible for the nutritional care of outpatients in the renal program. This may include patients on renal replacement therapy (hemodialysis and peritoneal dialysis), that may receive their therapy in-centre at one of WRHN’s satellite units or at home. Duties include comprehensive nutritional assessment, development and implementation of nutrition care plans which includes nutrition education and some nutrition support, while working within an interdisciplinary team. Additional responsibilities may include actively participating in quality improvement initiatives, being a contributing member of quality council(s) and the mentoring of dietetic practicum students. Contributes to ensuring there is a safe environment for all patients, staff, and visitors.
Position Requirements:
- Registered Dietitian, membership in the College of Dietitians of Ontario.
- Clinical experience with preference given to candidates with renal nutrition, adult education and nutrition support.
- Knowledge of formal and informal community services and resources an asset.
- Demonstrated ability to organize, prioritize, problem solve and respond to competing, unscheduled demands.
- Experience and skill in complex decision making processes, advancing best practice initiatives, and exploration of innovative models of care.
- Strong counselling skills and understanding of patient engagement philosophies.
- Excellent interpersonal, written and verbal communication, organization, and time management skills.
- Self-directed, organized, and diplomatic.
- Patient navigation focused.
- Demonstrated ability to function independently and as part of a multidisciplinary team.
- Experience with quality improvement an asset.
- Good computer skills.
