Job Description: Case Manager
Position Summary:
Under the supervision of the Case Manager Supervisor and Program Manager you will be responsible to work collectively within a team environment ensuring all the families needs are being met in a timely manner, following the practices and processes that support the mission, vision statements and goals of Makoon Transition Inc.
Responsibilities:
- To oversee families residing in Makoon Transition Inc. engaging families to build and meet their identified healing goals. To help them as they work alongside Child Family Services of reunification and assist in navigating the child welfare system.
- To be prompt and ready for work in a positive manner.
- Sign in and out daily for accountability of attendance and tracking of time.
- Maintain a professional relationship with all families and other collaterals internal and external of Makoon Transition Inc. not crossing any ethical boundaries.
- Follow all rules, policies and regulations enforced by Makoon Transition Inc being a positive role model.
- Able to adapt to work process, procedures and follow feedback from Case Manager Supervisor, CM Team Lead and Management Team.
- Communicating any concerns or issues to the Case Manager Supervisor, Program Manager, Family Support Work Supervisor and management while following the chain of command and communications process internal and external of Makoon Transition Inc.
- Strong communication skills with other units such as Wellness team, Addictions facilitation team, FSW team within the building to ensure there is no gap of services to clients and there is a strong collaboration to meet families needs.
- Use critical thinking, strength based, trauma informed approach when making choices that can impact families. Be mindful of the privilege and power of choices and be mindful of the short and long term outcomes.
- Protect the integrity and work culture of Makoon Transition Inc.
- Assessing client's physical and mental wellness needs, preferences, abilities and developing plans to improve. Completing client Assessment tools within a prompt manner while moving in to Makoon Transition Inc. to ensure all needs are met and gaps are identified.
- Communicating any concerns or issues to the Program Manager and Management immediately.Assessing clients’ physical and mental wellness needs, preferences and abilities and developing plans to improve.
- Working with clients, family and friend support networks, health professionals, EIA and other community collateral as needed to assist in advocating in a professional manner while assisting in lowering barriers for clients.
- Listening to clients’ concerns and providing counseling or intervention as required. Complete therapy referrals for clients and children from the FNIHB/Jordan Principle list of providers. Refer and encourage clients to connect with counselling sessions at Makoon Transition Inc. with Addictions Coordinator and Wellness Coordinators.
- Encourage and empower clients to attend programming for healing and wellness. Ensure they are following all rules and agreements signed at Intake during their interview process.
- Schedule systems meetings with clients, CFS and outside support systems to discuss strengths and areas of growth.
- Be adaptable and understanding with clients, while following the mission and vision of Makoon Transition Inc. Maintain knowledge of all new and or changing protocols pertaining to standards of practice at Makoon Transition Inc., drug testing protocols,/practice, Child Family Services Act, policy changes within Jordan’s Principle and or any other information that may affect the families.
- Recording clients’ progress through monthly progress reports which include accurate information based on shift summary documentation, wellness checks, your personnel assessment of the parent and child/children ensure they are submitted to the child welfare agency by the appropriate date each month.
- Assessing clients progress and encouraging in respectful/kind manner to clients areas of concerns. Ensuring the children's basic needs are met and there are no child protections or safety concerns. Any child protection concerns need to be addressed with management, the child welfare agency and or ANCR(Child and Family All Nations Coordinated Response Network) if after hours. If a child is in danger needing protection communication with all staff, increase wellness checks if needed and or remove children from harm's way if necessary.
- Encouraging clients to reclaim their voice while allowing them their space and autonomy to choose what is best for their family.
- If any concerns are documented and or observed by Family Support Staff and or voiced by the client ensuring as Case Managers there is proper follow up with the clients When doing progress reports with clients that eyes are laid on the children and assessing/addressing the concerns.
- Assess and recognize when families are ready to transition out of Makoon, communicate with the Case Manager Supervisor, Case Manager team along with Sam Management to assist in creating a solid and secure transition plan.
- Take part in on-call rotation, support staff and clients when emergencies arise, this may require you to attend the building if necessary.
- Respond to family’s questions or concerns with empathy and professionalism.
- Maintain a professional relationship with all families, child and family services agencies, community organizations and help branches.
- Assist in system navigation for clients and help lower barriers to better social, economical and health outcomes for clients.
- When responding to emergency or crisis situations and/or contact 911 and the Management Team. Complete incident report as needed, ensure all the proper management or collaterals are notified such as child welfare.
- Ensuring no drugs, alcohol or illegal substances enter the building.
- Provide direction and oversight of the Family Support Team Lead to make sure families needs are met.
- MUST have your own transportation to and from work. Drive clients on caseload as needed or if Family Support Worker are unavailable.
- Do not contribute or start Lateral Violence.
- Participate in debriefs daily, monthly staff meetings, family events, team building activities and clinical supervision with Case Manager Supervisor and residents meeting.
- Log in to and update client information and case plans on Makoon Transition Inc. Database.
- Other duties as assigned.
Qualification:
- Grade 12 education required, with a college or university education is preferred in the field of expertise, 3-5 years experience in Child Welfare.
- Valid Class 5 Driver’s Licence and access to a vehicle.
- CPR First Aid and de escalation training.
- Mental Health First Aid
- Non-Violent Crisis Intervention
- Strong verbal, written and communication skills to articulate care plans to clients, support staff and professionals.
- Sound critical thinking and problem-solving skills to assess clients, analyze feedback from health care and social workers to determine the best care plans.
- Able to follow the missions and vision statements of Makoon Transition Inc. for the success of the families.
- Knowledge of Google Workspace and Microsoft Office with the ability to navigate shared drives.
- Strong organized and time management skills are a must to manage multiple families.
- Must provide criminal record, adult and child abuse registry checks.
- Agree to random drug testing for illegal substance use and alcohol for work safety.
