About the Role:
At Moose Valley Regional Hospital, we are committed to delivering compassionate, coordinated, and high-quality care to every patient. We are looking for a Patient Care Manager (m/f/d) who can support the daily coordination of patient services and help our clinical teams deliver an excellent standard of care.
You will work closely with nurses, physicians, allied health professionals, administrative teams, patients, and families. This is a hands-on mid-level management position focused on patient care coordination, staff support, service quality, and efficient hospital operations.

Key Responsibilities
Patient Care Coordination: Coordinate daily patient care activities and help ensure patients receive timely, appropriate, and well-organized services.
Team Support: Provide day-to-day guidance to patient care staff and assist with operational issues, workload concerns, and service coordination.
Care Quality: Monitor patient care practices and support compliance with hospital policies, safety procedures, and quality standards.
Patient Experience: Respond to patient and family concerns professionally and coordinate appropriate follow-up with relevant departments.
Staff Scheduling: Assist with staffing coordination, shift coverage, workload planning, and daily department requirements.
Documentation: Review relevant patient care documentation and ensure required reports, records, and follow-up activities are completed accurately.
Interdepartmental Collaboration: Coordinate with nurses, physicians, allied health professionals, and administrative teams to support efficient patient services.
What We Are Looking For?
Experience: 3 to 5 years of experience in healthcare, nursing, patient services, healthcare administration, or a related field, including at least 1 year of team coordination or supervisory experience.
Education: Diploma or degree in nursing, healthcare administration, health services management, or a related discipline.
Leadership: Demonstrated ability to coordinate staff, manage daily priorities, and support healthcare teams in a fast-paced environment.
Communication: Strong written and verbal communication skills with the ability to work effectively with patients, families, healthcare professionals, and hospital management.
Organization: Excellent organizational and problem-solving abilities with the capacity to manage multiple patient care priorities.

Pay: $6,520.00-$7,600.00 per month
