We are seeking dedicated and professional Clinical Case Managers to join our team and contribute to meaningful outcomes for the individuals and communities we serve.
Position Summary
The Clinical Case Manager will primarily provide direct support, consultation, training, and supervision to home share providers; screen prospective care providers; complete home studies/initial licensing pieces and prepare for annual license reviews; provide assessments of client’s needs; and will be responsible for the overall development, implementation, and monitoring of intervention plans for clients on their caseload.
Key Responsibilities
- extensive experience working with Community Living disABILITY Services (CLdS), to primarily oversee a caseload of home shares for adults living with an intellectual disability.
- Additional case management experience with children/youth in the care of Child and Family Services would be an asset to support transitions of youth in care to adult services.
Qualifications
Required
- Experience working with Community Living disABILITY Services (CLdS), preferably in case managing and/or licensing home shares; additional experience working with children/youth with developmental, behavioral and emotional challenges is an asset.
- Experience working with children/youth/adults in CFS/CLdS, and their licensed support providers or foster/kinship families, and birth families.
- Excellent assessment skillsAbility to work independently and collaboratively in a team environment.
- Experience and ability to develop effective treatment/intervention plans.
- Commitment to working from a trauma-informed and strengths-based perspective.
- Strong understanding of attachment issues and needs, and the importance of attachment-based care providing
- Experience working collaboratively with different systems (Child Welfare, Adult Services, Justice, Education)
- An understanding of colonization, intergenerational trauma, Indigenous practices and traditions, and direct experience working with Indigenous clients.
- Excellent communication and interpersonal skills.
- Excellent written skills and demonstrated ability to effectively complete paperwork and meet established deadlines.
A Bachelor of Social Work Degree (or related degree/diploma in disabilities, psychology, etc.) and three years of experience working with clients and their caregivers is preferred. Successful applicants will have a clear Criminal Record Check, Child and Adult Abuse Registry Checks and Prior Contact Check and possess a valid driver’s license and a reliable vehicle.
