Compensation: $127,987 annually

Schedule: 5 days a week, including 1 evening per week and approximately 1 Saturday per month

The Humber River Family Health Team (HRFHT) is a non-profit organization providing interdisciplinary, patient-centred primary care across northwest Toronto. Our team works collaboratively to improve access to high-quality, inclusive primary care, chronic disease management, health promotion, and preventative services.

About the Position
HRFHT is seeking Nurse Practitioners(NPs) to provide comprehensive primary care while supporting our continued growth, patient attachment, and program development initiatives.
The NP will work at designated HRFHT sites and will play an important role in screening and assessing new patients, identifying their primary care needs, and supporting their attachment and rostering to participating physicians.
The NP will also contribute to the development, implementation, facilitation, and evaluation of clinical and community-based programs in response to identified patient and community needs.
Accountability
The position reports directly to the Supervisor, Clinical Services.
Key Responsibilities
- Conduct comprehensive and focused health assessments.
- Screen and assess prospective new patients as part of HRFHT's patient intake and attachment process.
- Support the rostering and attachment of eligible patients to participating physicians at designated HRFHT sites.
- Build and manage a caseload of patients in collaboration with HRFHT physicians and the interdisciplinary team.
- Diagnose and manage acute and chronic health conditions within NP scope of practice.
- Provide preventative care, health screening, immunizations, well-baby and well-woman care.
- Order and interpret appropriate laboratory and diagnostic investigations and prescribe treatments within scope.
- Refer patients to specialists, HRFHT programs, and community services as appropriate.
- Develop, implement, and facilitate clinical, health promotion, prevention, and community-based programs.
- Facilitate individual and group-based patient education and programming as required.
- Participate in program planning, evaluation, quality improvement, and identification of emerging service needs.
- Work collaboratively with physicians, nursing, allied health, administrative, intake, and community partners.
- Maintain accurate and timely documentation in the EMR and support patient education, self-management, and navigation.
- Perform other related duties as required.
Qualifications
- Current registration in good standing with the College of Nurses of Ontario as a Nurse Practitioner (Primary Health Care).
- Demonstrated experience providing comprehensive primary care across the lifespan.
- Strong assessment, clinical decision-making, and patient management skills.
- Experience with preventative care and chronic disease management.
- Experience developing, implementing, or facilitating clinical, health promotion, or community-based programs is an asset.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Strong communication, facilitation, organizational, and patient-engagement skills.
- Experience using an electronic medical record system is an asset.
- Demonstrated flexibility and ability to work in a growing, evolving primary care environment.
