Peerless Trout First Nation is seeking a compassionate and experienced Registered Nurse to take on a dynamic community-based role that blends clinical care, public health, homecare, patient advocacy, and health promotion. This is a permanent, on-site position in Peerless Lake, Alberta, and requires relocation to the community. As the Registered Nurse – Lead Community Health & Homecare Nurse, you will work closely with an LPN, Advanced Care Paramedics, local health staff, and PTFN leadership to deliver coordinated, culturally safe care. This role is ideal for an RN who enjoys variety, values strong patient relationships, and is confident working both independently and as part of a multidisciplinary team. From supporting homecare and public health clinics to helping community members navigate the healthcare system, you will play an important role in strengthening access to care and improving health outcomes for PTFN members.

RESPONSIBILITIES:
- Homecare Service Delivery: Provide essential homecare services when KTC homecare is unavailable, including coverage during weekends, after-hours, and two scheduled weekdays.
- Mobile Integrated Health Collaboration: Provide clinical collaboration and as-needed support to the Mobile Integrated Health (MIH) Unit, under the leadership of the Advanced Care Paramedics.
- Public Health & Clinics: Deliver comprehensive public health nursing services, including Maternal and Child Health clinics, Early Childhood Development (ECD) screenings, and immunization administration.
- Clinical Testing & IPC: Perform basic specimen collection and handling, including blood and urine samples, and oversee Infection Prevention and Control (IPC) practices within the Health Centre.
- Patient Navigation: Act as a patient advocate and navigator, assisting community members in accessing services, addressing barriers to care, and navigating provincial healthcare pathways.
- Community Capacity Building: Mentor, train, and support local non-clinical community staff in delivering culturally relevant wellness and health promotion programs.
- Strategic Alignment: Collaborate with PTFN leadership to ensure health initiatives reflect community needs, cultural values, and organizational health priorities.
QUALIFICATIONS:
- Active registration and in good standing with the College of Registered Nurses of Alberta (CRNA).
- Bachelor of Science in Nursing (BScN) is preferred.
- Demonstrated experience in homecare, public health, community health, or rural/remote nursing.
- Previous professional nursing experience within the Canadian healthcare system is required, with a strong understanding of the Alberta healthcare system, including AHS referral pathways, public health guidelines, and community-based care services.
- Demonstrated leadership, mentoring, or program coordination experience within a healthcare or community setting.
- Experience with immunization administration, maternal and child health, communicable disease programming, and public health initiatives considered an asset.
- Competency in venipuncture, blood and urine specimen collection, handling, and other basic clinical procedures.
- Knowledge of Infection Prevention and Control (IPC) standards, practices, and applicable healthcare protocols.
- Demonstrated knowledge of and commitment to trauma-informed, culturally safe, and patient-centred care within First Nations communities.
- Experience working with First Nations, Indigenous, rural, or remote communities considered a strong asset.
- Proven ability to work cohesively within a multidisciplinary team and flexibly share responsibilities with ACPs, LPNs, local health workers, and other healthcare providers.
- Ability to work independently, exercise sound clinical judgment, and respond effectively in community, home-based, and outreach settings.
- Strong communication, documentation, case coordination, patient advocacy, and interpersonal skills.
- Current Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) certification considered an asset.
- Ability and willingness to work evenings, weekends, and after-hours as required to support continuity of care.

- Copy of valid driver’s license and consent for driver abstract access
- RCMP Police Information Check (updated every 12 months)
