Position Title: Uniform Assessment System (UAS) Nurse
Position Type: Full Time
Reports To: Director of Patient Services
Position Summary:
The Uniform Assessment System (UAS) Nurse plays a critical role in assessing the clinical, psychological, and social needs of both prospective and existing members using the Uniform Assessment System for New York (UAS-NY). This position involves conducting initial and reassessments, developing care plans and ensuring compliance with New York State regulatory requirements.
Key Responsibilities:
- Performs detailed and systematic evaluations of patients using the UAS-NY Assessment to determine healthcare needs, functional status, and potential risks.
- Conduct initial and annual assessments, according to state guidelines, policies and protocols.
- Creates a proposed plan of care based on assessment findings, incorporating patient preferences and evidence-based practices.
- Completes all documentation during the home visit within 48 hours.
- Works with the quality review teams to ensure the accuracy and completeness of all necessary documentation in the Health Commerce System and electronic record system.
- Appropriately seeks guidance from assigned Supervisor and Manager, as needed
- Maintains knowledge of all applicable NYSDOH guidelines and organization policy and procedure in the completion of the UAS-NY Assessment.
- Ensures all NY-UAS assessments adhere to state and federal regulations, including documentation standards and reporting requirements.
- Provides education and support to patients and their families regarding the plan of care, healthcare needs, and available resources.
- Accurately records and update patient information in the UAS system, maintaining confidentiality and data integrity.
Qualifications:
- Education: Associate or bachelor’s degree in nursing (ADN or BSN) from an accredited institution.
- Licensure: Current NYS Registered Nurse (RN)
- Experience: Minimum of 1 years of nursing experience, with experience in care coordination, patient assessment, or case management preferred.
- Experience with the completion of a UAS-NY Assessments, preferred.
- Skills:
- Proficiency in completing NY-UAS Assessment.
- Strong analytical and problem-solving abilities.
- Excellent communication and interpersonal skills.
- Ability to work effectively both independently and as part of a team.
- Detail-oriented with strong organizational skills.
Work Location:
This position will require an in person visit to the patient’s home or health care facilities.
Salary & Benefits:
- Salary: Approximately $90,000 USD annually.
- Bonus: $5,000 USD onboarding bonus upon successful TN Visa acquisition and commencement of employment.
