Position Overview
The Licensed Practical nurse (LPN) is accountable for the delivery of direct nursing care for a group of Residents and provides leadership for the nursing team members who work under their supervision, in the assessment, planning, implementation and evaluation of direct resident care services.
Experience
- Previous experience in Long Term Care an asset
- Experience with chronic illness, geriatrics and complex care
Education (Degree/Diploma/Certificate)
- Graduate of an approved nursing educational program.
- Current registration with College of Licensed Practical Nurses of Manitoba (CLPNM).
Qualifications and Skills
Knowledge of:
- Completes assessments of the resident utilizing the Physical, Psychological, Spiritual, Cultural and Economic Domains, to assist in identifying individual health care needs.
- Performs ongoing assessments of the residents as their health status changes
- Develops an Integrated Care Plan utilizing data available including input from residents and families, and members of the Interdisciplinary Health Care Team.
- Documents applicable data in the Integrated Progress notes utilizing FOCUS based charting
- Organizes duties to ensure efficient care
- Participates in Emergency Codes as directed
- Participates in required Mandatory Education
- Demonstrated effective oral and written communication skills.
- Demonstrated critical thinking/problem-solving skills.
- Accountability to their actions
- Ability to maintain concentration with frequent interruptions.
- Other duties as assigned
Communication
- Responds appropriately and professionally to inquiries from Residents, families, and co-workers
- Communicates and documents accurately and in a timely manner
