We are currently seeking a Primary Health Care Nurse Practitioner.
This is a permanent full time (1 FTE) position starting immediately.
The Central Hastings Family Health Team is a group of collaborative health care professionals, located in the heart of Hastings County. We operate out of offices in Madoc, Marmora and Gilmour. The Nurse Practitioner has an integral role in the care of the patients of our team, and assists in achieving the organization’s vision, mission, values and goals. The NP will practice within and to their full scope of practice in patient care and the delivery of FHT programs working collaboratively within an interdisciplinary healthcare team.
Position Description
The Nurse Practitioner (NP) is an integral member of our Family Health Team, providing comprehensive, patient-centered primary care across the lifespan. In addition to delivering high-quality acute episodic care, chronic disease management, and preventative health services, the successful candidate will contribute to the implementation of new and existing programs and quality improvement initiatives. This role combines program development with a satisfying primary care practice delivered across all three of our health care sites, in close collaboration with our interdisciplinary team.
Main Responsibilities
- Provide primary health care to patients including health promotion, disease prevention and chronic disease management
- Provide episodic care to patients of the collaborating Physicians of the Family Health Team presenting with acute illnesses
- Participate in the development and delivery of a broad range of health promotion and chronic disease management programs and patient education
- Collaborate with other multi-disciplinary team members and external partners on programming and day to day issues
- Provide input for the quality improvement refinement / progress of the FHT’s program planning
Qualifications and Experience
- Current registration with the College of Nurses in Ontario in the Extended Class RN(EC)
- New Graduates are welcome to apply
- Excellent health assessment skills, knowledge and proficiency in evidence-based methods
- Excellent written and oral communication skills
- Be proficient in EMR use and documentation – Telus PS experience an asset but not a requirement.
- Ability to work collaboratively as a member of FHT
Compensation & Benefits
- $60.33 per hour – 40 hours per week paid bi-weekly pay period
