Role: Reporting to the Lead Service Coordinator, the Service Coordinator provides case management to assist adult clients with identifying goals and obtaining the clinical and community support they need to reintegrate and regain independence.
Qualifications and competencies:
- Diploma or degree in human services or health field
- RSW, PHEc, and/or other professional designations are an asset
- Experience: Case management or program coordination, human services work
- Up-to-date and clear criminal record check
- Possess a valid driver’s license and access to own vehicle with 2 million liability insurance
- Verbal, listening, and written communication skills
- Conveys respect when listening and responding to clients, practices self-awareness and regulation of emotions to use them productively.
- Problem solving and decision-making skills
- Accesses resources, information and technology from other areas and uses it to analyze issues, resolve problems and improve performance
- Empathetic and responsive
- Ability to view social issues from a variety of lenses and treats individuals from all walks of life with respect, and compassion
- Collaborates and communicates effectively with staff team, clients, and other stakeholders, and takes initiative in completion of day-to-day work
- Experience working with adults with brain injuries and/or disabilities is an asset
Major Responsibilities:
- Case management and coordination of services for individuals and families impacted by brain injury
- Creates and executes realistic work plans, manages multiple tasks and meets deadlines through prioritization, goal setting, organization and teamwork
- Conduct a thorough initial interview with new clients to gather appropriate information that will inform their service plan
- Communicate with clients to obtain general information as it relates to the etiology, symptoms, and treatment of injury, as well as communication, behavioral, cognitive, physical, psychosocial and family issues
- Provide referrals for other community resources in a timely fashion
- Properly discharge clients upon goal and service completion
- Record activities in applicable database
- Complete quarterly and annual reports by established deadline
- Collaborate with other agencies from across the province of Alberta when required
- Participate in provincial brain injury events
- Actively promote education and information about living with acquired brain injury to clients, their families, community service providers.
- Provide brain injury information to clients and their support networks
- Act as a representative of BCC at public events as required
Position Detail:
- 1.0 Full-Time Equivalent; 1 year contract with the possibility of extension
- Annual salary commensurate with experience
- Work week of 37.5 hrs which may involve occasional evenings or weekends
- Ability to join BCC's group benefit plan, 50% employer paid

- Annual provision of 15 days of paid vacation as well as two weeks of paid office closure time at Winter break.
